Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding k/2 -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€ms installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

Suspect ACBM were grouped into hornogencous areas based on physical characteristics
and functional application. Homogeneous areas were divided into one of three categories
based on as designated by Colorado Regulation 8 and the EPA.
Sampling was conducted per Colorado Regulation 8 protocol, specifying the minimum
collection of
0 3 samples of surfacing materials less than 3,000 square teet in size.

Laboratory analysis in this survey detected trace asbestos in the original interior plaster
walls and in a caulking used to seal, a crack the stucco on the south side of the exterior.
Neither of these materials are regulated by the CDPHE or the EPA and should they be
damaged or require demolition may be disposed of as normal construction waste,

The plaster contained a trace (< 1% asbestos) of chrysotile asbestos. This material is not
regulated by the State of Colorado or the US EPA. 0SHA regulations for worker protection
apply and appropriate measures should be used during demolition activities.

Of the 4 samples collected representing four separate painted surfaces LBP was not detected,

Sample locations were selected in a random manner. The (number off samples were collected of
each homogeneous material identified as suspect ACBM in accordance with the referenced
Colorado Regulation 8 ACMB protocol and Colorado Regulation 19 L, BP Protocol.
The following building materials were identified as suspect ACBM and sampled for
asbestos content or presumed ACBM:

A total of 4 chip samples were collected from 4 identified painted surfaces on painted surfaces on
the wood-work, plaster walls. No lead based paint was detected. Please see 'rate # 2 for chip-
sample locations and results.
SECTION 10 INACCESSIBLE AREAS:
Reasonable efforts were made to locate and inspect concealed areas in accordance with 40 CFR
763. Only the areas and the materials listed were surveyed as part of this inspection.

SECTION 2 METHODOLOGY:

Suspect ACBM were grouped into homogeneous areas based on physical characteristics
and functional application. Homogeneous areas were divided into one of three categories
based on as designated by Colorado Regulation 8 and the EPA.
Sampling was conducted per Colorado Regulation 8 protocol, specifying the minimum
collection of,
* 3 samples of surfacing materials less than 3,000 square feet in size.
• 5 samples of surfacing materials between 1,000 and 5,000 square feet in size, and
0 7 samples of surfacing materials greater than 5,000 square, feet in size.
• 1+ sample/s to characterize miscellaneous materials as appropriate
A material is considered by the Environmental Protection Agency (EPA) and the Colorado
Department of Public Health and Environment (CDPliE) to be asbestos-containing material
(ACM) if at least one sample collected from the homogenous material has asbestos present in a
concentration greater than one percent (> I %).

LABORATORY ANALYSIS
SECTION 3 RESULTS /CONCLUSIONS
ACBM Sample Findings
Laboratory analysis in this survey detected trace asbestos in the original interior plaster
walls and in a caulking used to seal a crack the stucco on the south side of the exterior.
Neither of these materials are regulated by the CDPFIE or the EPA and should they be
damaged or require demolition may be disposed of as normal construction waste.

The plaster contained a trace (< 1% asbestos) of chrysotile asbestos. This material is not
regulated by the State of Colorado or the US EPA. OSFIA regulations for worker protection
apply and appropriate measures should be used during demolition activities.

LBP Sample Findings
A total of 4 chip samples were collected from 4 identified painted surfaces on interior wood-
work, and walls that have been impacted by demolition activities.
Of the 4 samples collected representing four separate painted surfaces L was not detected.
Analysis by Atomic Absorption (AA) of all L samples collected reported lead levels to be
less than 0.5% lead

Sample locations were selected in a random manner. The (number of) samples were collected of
each homogeneous material identified as suspect ACBM in accordance with the referenced
Colorado Regulation 8 ACM B protocol and Colorado Regulation 19 LBP Protocol.
The following building materials were identified as suspect ACBM and sampled for
asbestos content or presumed ACBM:
