I . This permit was issued based on the information provided in the permit application and accompanying flans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a change of the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, any violation of any provision of any
applicable code or any ordinance or regulation of this jurisdiction. Approval of work is subject to Held inspection.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online--do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 3-5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

9/22/08 Memo was sent to Ken concerning Steve Blakesley's visit to the office,
regarding electrical requirements for 6265 W. 46" Piace. Requirement to
upgrade is red-lined on plans, and was discussed as part of conversation
between John Sturken and Mike Villano last week.
9/23/08 I accompanied Kirk to his inspections this morning. First was 6265 W.
46th Piace for a framing inspection. We were met by Jason Brady, who freely
spoke to us about the project. He stated that as apart of the project, he would be
changing most or all of the receptacles in the house, and that he was also
intending to add additional receptacles. This work is not noted on the submitted
plans. During a walk of the residence, noted items include new or demolished
plumbing in the bathrooms, laundry room, and kitchen; abundant electrical
materials, including new boxes, wire and plugs, all stored in the kitchen; the
water heater has been removed and was not present inside the residence. The
water heater is not indicated for replacement on the re-submitted and approved
plans. Mr. Brady also stated that to his knowledge, there is intent to replace the
furnace in the crawl space. He stated that he was present for the conversation
between John Sturken and Mike Villano, and he confirmed that Mr. Villano
informed Mr. Sturken of the kitchen electrical requirements, and that the electrical
service and masthead are adequate, and that the current panel is too small to
accommodate the necessary circuits. He also confirmed that there is only one
opening remaining in the existing panel, and that the air-conditioning is not yet
wired into the panel. Pictures of the laundry, kitchen and baths were taken.
We then went to perform a final plumbing re-inspection and initial final building
inspection at 6914 W. 48`h Avenue. The plumbing inspection was approved.
Items noted on the final building inspection included insulating the attic access
cover, sealing around the exterior of the newly installed patio doors, the variation
of stair risers in excess of the allowable 3/8", and the need to insulate exterior
wails and remove insulation in the crawl space plenum. We were questioned by
Kim about this, who first asked if plenum meant a lot, then asked "What is a
plenum?" We explained a plenum and the requirements, and she stated that she
thought that she could get these items done today. No conversation with Steve
occurred.

FLOORS
SECTION R501
GENERAL
01.1 ApplIcation.'The provisions of this chapter shall con-
~l the design and construction of the floors for all buildings
including the floors of attic spaces used to house mechanical
ad/or plumbing fix[ures and equipment.

accommodating all loads according to Section R301 and of
nnsmittmg the resulting loads to the supporting strucmral ele-

SECTION R502
WOOD FLOOR FRAMING
002.1 Identi6cation. Load-bearing dimension lumber for
joists, beams and girders shall be identified by a grade mazk of a
Wmber grading or inspec[ion agency that has been approved by
an accreditation body that complies with DOC PS 20. In lieu of
a grade maz'k, a cer[ifica[e of inspection issued by a lumber
grading or inspec[ion agency meeting the requirements of this
section shall be accep[ed.
R502.11 Preservatively treated lumbee Preservatively
treated dimension lumber shall also be identified as required
by Sectiou R3191.
R5021.2 Blocking and subflooring. Blocking shall be a
minimum of udliry grade lumber. Subfloocing may be a
minimum of utiliry grade lumber or No. 4 common grade
boazds.
R5021.3 End-jointed lumbet Approved end-jointed lum-
ber identified by a gcade mark conforming to Sec[ion
R501.2 may beused interchangeably with solid-sawn mem-
bers of the same species and grade.
R5021.4 Prefabricated wood I-joists. Structura] capaci-
ties aod design provisions foc prefabricated wood I joists
shall be established and monitored in accordance with
ASTM D 5055.
R502.1.5 S[ructural glued laminated timbers. Glued
laminated timbers shall be manufactuced and ideutified as
cequired in AITC A190.1 and ASTM D3737.
R502.2 Design and construction. Floors shall be designed
and constructed in accordance with the provisions of this chap-
ter, Figure R502.2 and Sections R319 and R320 orin accor-
dance with AF&PA/NDS.
R502.21 Decks. Where supported by attachmeut [o au ex-
terior wall, decks shall be positively anchored to the primary
strucmre and designed for both vertical and lateral loads as
applicable. Such attachment shall not be accomplished by

the ase of roeuails or nails subject [o withdrawal. Where
positive connec[ion to the primary building structure cannot
be verified during inspection, decks shall be self-support-
ing. For decks with cantilevered framing members, connec-
[ions to exterior walls or other framing members, shall be.
designed and coustmcted to resist uplifr resulting from the ;
full live load specified iu TableR301.5 acting on the cantile- !
vered portion of the deck.
R502.3 Allowable joist spans. Spans for floor joists shall be in
accordance with Tables R50231(1) and R50231(2). For
other grades and species and for other loading conditions, refer
to the AF&PA Span Tables for Joists and Rafters.
R502.31 Sleeping areas and attic joists. Table
R502.3.1(1) shall be utilized to de[ermine the maximum al-
lowable span of floor joists that sapport sleeping areas and
attics that are aceessed by meaus of a fixed staieway pro-
vided that the design live load does not exceed 30 psf (1.44
kN/m2) and the design dead load does not exceed 10psf
(0.48 kN/m2). The allowable span of ceiling joists that sup-
port attics utilized for limi[ed storage or no storage shall be
determined in accordauce with Section R802.4.
R502.3.2 Other floor joists. Table R5023. L(2) shall be uti-
lized to determiue the maximum allowable span of tloor
joists that suppoxt all azeas of the building, other than sleep-
ing and attics, provided that the design live load does not ex-
ceed 40 psf (1.92 kN/m2) and the design dead does not
exceed ] 0 psf (0.48 kN/m').
R502.3.3 Floor cantilevers. Floor cantilever spans shall
❑ot exceed the nominal depth of the wood floor joisL Floor
eantilevers constructed in accordance with Table
R50233(1) shall be permitted when supporting a
light-frame bearing wa11 and roof only. Floor cantilevers
supporting an exterior balcony are permitted to be con-
structed in accordance with Table R50233(2).
R502.4 Joists under bearing partitions. Jois[s under parallel
bearing partitions shall be of adequate size to support the load.
Double joists, sized to adequately support the load, that are sep-
arated to permit the installa[ion of piping or vents shall be full
depth solid blocked wi[h lumber notless Chan 2 inches (51 mm)
in nominal thickness spaced not more than 4 feet (1219 mm) on
center. Bearing partitions perpendiwlar ro joists shall not be
offset from supporting girders, walls or partitions more than the
joist depth unless such joists ace of sufficient size ro carry the
addi[ional load.
R502.5 Allowable girder spans. The allowable spans of gird-
ers fabrica[ed of dimension lumber shall not exceed the values
set foRh in Tables R502.5(1) and K502.5(2).

and shall apply to all roof/ceiling portions of the building

ing insulation to be obtained ovei [he wall top plate, R-30

N1102.1.3 Opaque doors. Opaque doors sepazating condi-
tioned and unconditioned space shall have a maximum
U-factor of 0.35. One opaque door shall be permitted to be
exempt from this U-factor requirement.
N1102.1.4 Floors. The reqaired R-value in Table N 1102.1
shall apply to all floors, excepC any individual flooc assem-
bly with over 25 percent of its conditioned floor area ex-
posed directly to outside air shall meet the R-value
requiremen[ in Table N 1102.1 for °Ceilings."
Nll021.5 Basement walls. When the basement is a condi-
tioued space, the basement walls shall be insulated in acwr-
dance with Table N1102.1. When the basemen[ is no[ a
condirioned space, either the basement walls or the ceilings
separating the basement from conditioned space shall be in-
sulated in accordance with Table Nl ] 02.1. When insulating
basement walls, the reyuired R-value shall be applied from
[he top of the basement wall to a depth of 10 feet (3048 mm)
below grade or to the top of the basemen[ floor, whichever is
less.
N1102.1.6 Slab-on-grade tloors. For slabs with a[op edge
above finished grade or 12 inches (305 mm) or less below
Yinished grade, the reqaired R-valae in Table N 1102.1 shall
be applied to the outside oF the foundation or the inside of
the foundation wall. The insulation shall extend downwazd
from the top of the slab, ordownward to the bottom of the
slab and then horizontally in either direcdon, until the dis-
tance listed in Table N 1102. L is reached.
When installed between the exterior wall and the edge of
the interior slab, the top edge of the insulation shall be per-

demonstrated by (a) using Internatiorwl Energy Conser-
vation Code Section 5022.2 or Internarional Energy
Conserva[ion Code Chapter 4 with the actual slab insula-
tion R-value iu Table N 1102.1, or (b) using International
Energy Conservndan Code Section 5022.4.
N11021.7 Crawl space walls. Where the floor above the
crawl space is uninsulated, insulation shall be installed on
crawl space walls when the crawl space is not vented [o out-
side aic The required R-value in Table N1102.1 shall be ap-
plied inside of Che crawl space wall, downward from the sill
plate to the ex[erior finished grade level and then vertically
and/or horizontally for at least an additional 24 inches (610

N11021.8 Masonry veneec For extenor foundation insu-
lation, that horizontal portion of Ihe founda[ion [hat sup-
ports amasonry veneer shall not be required to be insulated.
N1102.1.9 Protection. Exposed insulating materials ap-
plied to the exterior of foundation walls shall be protected.
The protection shall extend a[ least 6 inches (152 mm) be-
low finished grade level.
N11021.10 Air leakage. All joints, seams, penetrations;
site-bailt windows, doors, and skylights; openings between
window and door assemblies and their respective jambs and
framing; and other sources of air leakage (infiltration and
exfiltration) through the building thermal envelope shall be
caulked, gasketed, weatherstripped, wrapped, or otherwise
sealed [o limit unwntrolled air movement.
N1102.1.11 Recessed lighting Sxtures. When installed in
the building emelope, recessed lighting fixtures shall meet

N1102.2 Maximom solar heat gain coefficient for fenestra-
tion prodacts. The acea-weighted-average solar heat gain co-
efficien[ (SHGC) for glazed fenestration installed in locations
with 3,500 or fewer heatiug degree days shall not exceed 0.40.
N1102.3 Fenestration exempNon. Up to 1 percent of the [otal
glazing azea shall be exempt from (7-facCOr requirement.
N1102.4 Replacement fenestration. Where some or all of an
existing fenestration unit is replaced with an eneirely new re-
placement fenestration product, including frame, sash and
glazed portion, in an existing building, the replacement fenes-
tradon product shall have a U factor that does not exceed the
"Maximum fenestration U factor" and an SHGC that does not
exceed the "Maximum feuestration SHGC° in Table N 1102.1

[E] N11031 Heating and air conditioning appliance and
equipment performance. Performance of equipment lis[ed in
Table N11031 is covered by preemptive Federal law. Appli-
ances and equipment not listed in Table N1103.1 shall meet the
minimum efficiency requirements of Section 503.2 of the In-
ternarional Energy Conservation Code.
N1103.2 Controls. At least one thermostat shall be provided
for each separate heating, cooling, or combina[ion hea[ing and
cooling system. Hea[ pumps shall have controls [ha[ preven[
supplementary electric resistance heater operation when the
heating load can be met by the heat pamp alone. Supplemen-
tary heater operation shall be permitted during outdoor coil de-
frost cycles not exceeding 15 minutes.
N1103.3 Duct insulatioa All portions of the air distribution
system shall be installed in accordance with Section M1601
and be insulated to an installed R-5 when system components
aze located within the building but ou[side of condilioned
space, and R-8 when loca[ed outside of the building. When lo-
cated within a building envelope assembly, at least R-8 shall be
applied between the duct and that portion of the assembly fur-
thest from conditioned space.
Exception: Exhaust air ducts and portions of the air distd-
bution system wi[hin appliances or equipment.
N1103.4 Duct sealing. All ducts shall be sealed in acwrdance
with Section M76013.1.
[E] N1103.5 Piping insulation. All mechanical system piping
shall be insalated in accordance with Table Nll03.5.
Exceptions: Piping installed within appliances and equip-
men[ or piping serving fluids between 55°F (13°C) and

M1601.2.1 Duct insulation materials. Duct insulation ma-
terials shall conform to the following requirements:
1. DucC coverings and linings shall have a flame-spread
index no[ grea[er than 25, and a smoke-developed in-
dex not greater than 50 when tested in accordance
with ASTM E 84. Specimens tested in accordance
with ASTM E 84 shall be representative of the final
composite material.
2. Duct coverings and linings shall not flame, glow,
smoldec or smoke when tested in accordance with
ASTMC 411 at the Cemperature to which they aze ex-
posed in service. The test temperamre shall not fall
below 250°F (121°C).
3. External duct insulalion and factory-insulated flexi-
ble ducts shall be legibly printed or identified at inter-
vals not greater than 36 inches (914 mm) with the
name of the manufacturer; the thermal resistance
R-value at the specified installed thickness; and the
flame spread and smoke-developed indexes of the
composite materials. All duct insulation product
R-values shall be based on insulation only, excluding
aic films, vapor retarders or otbec duc[ components,
and shall be based on tested C-values at 75°F (24°C)
mean temperature at the insCalled [hickness, in accor-
dance with recognized industry procedures. The in-
stalled thickness of duct insulation used to determine
its R-values shall be determined as follows:
3.1. For duct board, duct liner and factory-made
rigid ducts not normally subjected to com-
pression, the nominal insulaGOn thickness
shall be used.
3.2. For duct wcap, the installed thickness shall be
assumed to be 75 percent (25-percent com-
pression) of nominal thickness.
33. For fac[ory-made flexible air duc[s, the in-
stalled thickness shall be determined by divid-
ing the difference between the actual outside
diame[er and nominal inside diame[erby two.
M1601.2.2 Vibra[ion isolators. Vibration isolators in-
stalled between mechanical equipment and metal duc[s
shall be fabricated from approved materials and shall not ex-
ceed 10 inches (254 mm) in length.
M1601.3 Installation. Duct installation shall comply wi[h
Sections M16013.1 through M1601.3.6.
M1601.3.1 Joints and seams. Joints of duct systems shall
be made substauUally airtight by means of tapes, mastics,
gaske[ing or other approved closure systems. Closure sys-
tems used with rigid fibrous glass ducts shall comply with
UL 181A and shall be marked "181 A-P" for pressure-sensi-
tive tape, "181 A-M" for mastic or "I81 A-H" for heat-sensi-
tive tape. Closure systems used with flexible air ducts and
flexible air counectors shall comply wiCh UL 181B aod shall
be mazked "181B-FX" for pressure-sensitive tape or
"18113-M" for masdc. Duct connections to flanges of air
distribution system equipment or sheet metal fittings shall
be mechanically fasteued. Crimp joints for round ducts shall
have a contact lap of at least 1.5 inches (38 mm) and shall be

M1601.3.2 Support Metal ducts shall be supported by
0.5-inch (12.7 mm) wide IS-gage metal straps or 12-gage
galvanized wire atintervals not exceeding 10 feet (3048
mm) or other approved means. Nonmetallic ducts shall be
suppor[ed in accordance with the manufacturer's installa-
tion instructions.
M1601.3.3 Fireblocking. Duct insCallations shall be
fire-blocked in accordance with Section R602.8.
M1601.3.4 DucY insulatioa Dact insulation shall be in-
stalled in accordance with the following requirements:
1. A vapor retarder having a masimum permeance of
0.05 perm [(2.87 ng/(s . m2. Pa)] in accordance with
ASTM E 96, or aluminum foil with a minimum [hick-
ness of 2 mils (0.051 mm), shall be ins[alled on the ex-
terior of insalation on cooling supply ducts that pass
through nonconditioned spaces conducive to conden-
sation.
2. Exterior duct systems shall be protected against the
elements.
3. Duct coverings shall no[ penetrate a fireblocked wall
or floor.
M1601.3.5 Factory-made air ducts. Factory-made air
ducts shall not be installed in or on the grouud, in tile oc
meCal pipe, or within masonry or concrete.
M 1601.3.6 Duct separation. Dacts shall be insWlled with
at least 4 inches (102 mm) separation from earth except
where they meet the requirements of Section M1601.1.2.
M1601.3.7 Ducts located in garages. Ducts in garages
shall comply with the [equirements of Section R3091.1.
M1601.3.8 Flood hazard areas. In areas prone to flooding
as established by Table R3012(1), duct systems shall be lo-
cated or installed in accordance with Section R323.1.5.
M1601.4 Under-floor plenums. An under-floor space used as
a supply plenum shall conform to the requirements of this sec-
[ion. Fuel gas lines and plumbing waste cleanou[s shall not be
loca[ed within the space.
M1601.4.1 GeneraL The space shall be cleaned of loose
combus[ible materials and scrap, and shall be tightly en-
closed. The ground surface of the space shall be covered
with a mois[ure barrier having a minimum thickness of 4mils (0102 mm).
M1601.4.2 Materia►s. The underfloor space, including t
sidewall insulation, shall be formed by materials havu!~,:

I received a visit from Steve Blakesley today concerning the above property and the Division's
requirement that the kitchen electrical appliances be made to comply with current codes.
Section E3301.2 of the 2003 IRC references NFPA 70 (2005 NEC) and the applicability of that
code to electrical alterations and installations. Section E3301.4 requires that any alteration
comply with current codes. Alteration is defined in the code as °`the...reconfiguration or
extension of any system, or the installation of any additional equipment.°' Extensive Alteration
is defined as existing when "the total area of all of the wark areas included in alteration exceeds
50 percent of the area of the dwelling unit." Further, the 2005 NEC, Article 406.4 (D) defines
requirements for the replacement of receptacles.
An addition to the permit was requested for the addition of an air-conditioning unit, and Mr.
Blakesley has stated that receptacles are being replaced in the kitchen and bathrooms. The
addition of the air-conditioning constitutes an alteration due to "the installation of additional
equipment." Additionally, when work areas include the kitcllen and laundry areas, code requires
the laundry to be "installed on an independent circuit." Article 406.4 (D) (2) states tbat
"ground-fault circuit-interrupter protected circuits shall be provided where replacements are
made at receptacle outlets that are required to be so protected elsewhere in this Code." The
addiYion of the air-conditioning, the replacement of receptades, and the inclusion of the laundry
room as a work area all require compliance with current codes, and as a result, require the
addition of circuits to the panelboard. The requirement to upgrade the kitchen electrical to
comply with current codes is clearly indicated on the approved plans returned to Mr. Blakesley
during the permit issuance process. Any additional upgrades of the panelboard, wiring or
service prompted by the addition of appliances or replacements of outlets were unknown based
on the information provided by Mr. Blakesley at the time of application, and thus, would be his
responsibility to anticipate and accommodate.

discuss the project at 6265 W. 48th Avenue, which I accommodated. He stated that he had
some questions concerning code requirements for electrical work on the project. He stated that
his electrician, John Sturken of JHS Electric, met with Mike Villano, staff electrical inspector, at
the project to consult with Mike concerning the code requirements for the project, and that this
consultation resulted in the need for a significant change order to WR2020. He stated that his
electrician was told that because the kitchen was required to meet code (which had already
been noted on the approved plans), this prompted a need for a change of the electrical
service, which was not possible without changing the masthead at the service entrance, all of
which resulted in the change order request to WR2020. These items or similar items are
included in a proposal by JHS Electric dated 9/15/08. However, the consultation between Mike
Villano and John Sturken occurred on 9/16/08. Mr. Blakesley then stated that his reason for
the meeting with me was to ask me for a favor. He requested that I make a statement
declaring that the items are not required by code, but are warranted based on issues of safety.
He stated that because Rob Osborn personally disliked me, he would deliberately deny any
changes orders based on code requirements in an effort to create a dispute with the Building
Division, but that if it was for reasons of safety, it would be much more Iikeiy that Mr. Osborn
would approve the change order.
I questioned Mike Villano about the consultation and was told that he and John Sturken waiked
the project, and very few items were noted by Mike and written down by Mr. Sturken during the
consultation. Those items were:
• The installation of the new air-conditioning and furnace would require the
addition of a new circuit
. The kitchen would be required to be compliant with current codes for electrical
installation, including the addition of circuits to accommodate the new GFCI
outlets, the dishwasher and the disposal
• The existing panel is too small to accommodate the new circuits, therefore a new
panel would be required. However, Mr. Sturken was specifically told that the
electrical was capable of accommodating the new panel without a service
change. No discussion of the masthead occurred at any time.
. The wire feed from the main panel to the sub-panel in the garage is unsecured
and unprotected, and is required to be placed in conduit or secured and
protected in a code-compliant fashion

First and foremost, this appears to be a deliberate attempt by Mr. Blakesley to misrepresent
the items noted during the consultation, and to misrepresent to Mr. Osborn the requirements
set forth in the building and electrical codes, and by the Building Division. I also believe that
Mr. Blakesley's statement concerning Mr. Osborn's desires to create a dispute with the
Building Division is a deliberate attempt to poison an already contentious relationship between
the Division and WR2020 for his own personal gain.
Additionally, the installation of the new furnace indicated on the JHS Electrical proposal and
the air-conditioning unit observed by Mr. Villano are not included in the scope of work on the
approved and issued permit. As a part of the review process, a comment letter was sent to Mr.
Blakesley on 8/20/08 requesting that he provide clarification of "proposed new conditions...
and other pertinent details of work in affected areas," due to the ambiguity of work information
provided. Neither his original nor his re-submitted plans or application indicate the installation
of these systems, thus, once again, Mr. Blakesley has performed work outside the scope of his
permit, in the exact same manner as the Dover Street project.
Finally, the proposal from JHS Electric is addressed to Brady Builders of Arvada, which is
owned by Jason Brady, who is the individual that was working as Mr. Blakesley's "employee."
Solicitation of proposals by Brady Builders for electrical subcontract work of this nature is
outside of the scope of normal "employee" conduct, is outside of the scope of the Class 4
license granted to Mr. Brady, and constitutes conduct requiring a general contracting license. It
is not permissible under Municipal Code to use the license of another firm to avoid licensing of
your own firm.
Mr. Blakesiey continues to operate in violation of numerous provisions of the adopted building
codes and responsibilities of licensees, repeats the same violations perpetrated on other
projects despite previous citation, discussion of the issues, and promises of adherence to
agreements, and deliberately misrepresents facts and conversations in an effort to deflect or
obfuscate his digressions. It is my suggestion that the license granted to Brady Builders be
revoked, and that, if the installation of the air-conditioning or furnace units is confirmed, the
license granted to Orion Development be suspended or revoked, and that municipal citations
be issued to Mr. Blakesley.

On August 6, 2008, the Building Division issued permit #080883 to Steve Blakesley of Orion
Development for window replacement at 6265 W. 46"' Place. Orion Development is a general
contractor licensed with the City of Wheat, exempted from Municipal Code Section 5-125
requirements for maintenance of Warker's Compensation based on execution of a written waiver
stating that the firm has no employees other than the owners.
On August 6, 2008, work was observed at 6265 46`h Place that was being performed by someone
other than Mr. Blakesley. I and Combination Inspector Kirk Cadotte amved at the site as Mr.
Blakesley was pulling away. We requested entrance to the structure, which was granted by the
individual working the on site, who identitied himself as Jason Bradley of Bradley Builders. I
questioned him as to whether he had a City of Wheat Ridge contractors' license, and he stated
that he did not. He stated that he was working for Mr. Blakesley under his license, and had been
told by Mr. Blakesley that he would not need a license if he was to work in this fashion. Mr.
Blakesley has been found to be employing unlicensed contractors on previous projects within the
City of Wheat Ridge, each of whom stated that Yhey were working under Mr. Blakesley's license.
Mr. Blakesley was previously notified of the municipal code requirements for licensing of
subcontractors and insurance requirements for employees.
Additionally, demolition work has been initiated at the address which is believed to be part of a
larger project for which the required permits have not been obtained. Photographs indicate that
drywall, insulation, and cabinets had been removed. Kirk Cadotte also observed that plumbing
fixtures had been removed. This violation constitutes the fifth violation by Mr. Blakesley in less
than a year of City of Wheat Ridge Municipal Code Section 5-85 IRC Section 105.1 requiring
that permits be obtained prior to performing work.
Based on the above violations and history, it was requested that municipal summonses be issued
to Mr. Blakesley by Heather Johnson of the City of Wheat Ridge Communiry Services Division.
