2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3, If this permit expires, a new permit may be required to I obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped
off to the Permit Desk at the City of Wheat Ridge. The report MUST BE SIGNED by the
Homeowner.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

TERMS AND CONDITIONS
1 All contracts subject to review and final approval by TruBuilt Construction, Inc.
2 All insurance contracts fully contingent on insurance approval of scope and price. In the event that
homeowners insurance coverage is wholly denied. contract is null and void In the event that insurance
scope and pricing is different than contract, TruBuilt Construction. Inc is responsible for completion of
and entitled to payment for ONLY scope and price as approved by insurance carrier Customer may
elect to have additional work done on a time and materials basis or to not have the work done
3 Supplemental claims or additions in scope and price which are secured on behalf of the homeowner from
the insurance company as part of an insurance claim will become part of this agreement.
4 Owner(s) named to this contract hereby authorize release to the project manager named in this contract
any and all information or documentation requested by project manager in reference to this contract,
including information requested from insurance carriers adjusters attorneys. mortgage companies, banks,
real estate agents, underwriters. title companies lien holders, and assigns.
5 Owner(s) named in this contract hereby authorize secondary assignment of any and all trades included
as part of the contract at the discretion of the project manager named in this contract Project manager
will secure subcontractors to affect the same, where applicable
6 All work will be completed within 90 calendar days of final insurance approval or contract date, whichever
is later, weather permitting.
7 Pavment terms Contractor shall collect all ACV monies upon delivery of roofing materials to customer's
property. Contractor shall collect RCV funds upon disbursement from the insurance carrier.
8 Contractor(s) shall not be liable for any damages to the interior of the building. except as caused by leakage
9 Contractor(s) shall not be liable for minor damages to exterior of building which typically occur during the
performance of a roof installation, including damage to gutters.. facia, exterior paint, deterioration of pre
existing skylights, decking and evaporative coolers.
0 This contract constitutes the entire understanding of the partes, and no other understanding, collateral
or otherwise, shall be binding unless in writing and signed by both parties.
9 1 Replacement of deteriorating decking, facia, roof jacks. ventilators. flashing, or other materials unless
otherwise stated in this contract, are not included and are subject to additional charges on a time and
material basis if requested by the homeowner.
12 This agreement shall expire one year from the contract date unless extended in writing by the project
manager, contractor(s), or if the Insurance company's approval is still penaing
13 If the contract is breached or cancelled by the homeowner at any time after any work has commences,
including but not limited to meetings with adjusters discussions with insurance carriers or adjusters
regarding scope and price, creation of project diagrams/drawings, material delivery. or performance of any
completed work. a cancellation fee equal to 15% of the value of the contract applies, in addition to charges
for completed work and/or charges for material stocking fees. where applicable.
14 Any representations, or other communications, not written in this Contract are agreed to be immaterial
and not relied on by either party
15 Manufacturers Warranties are provided by the material manufacturer(s) and are subject to the terms and
conditions established by the manufacturer.
16 \Norkrnanshiu warranty provided by TruBuilt Construction. Inc covers any defects in workmanship
for a period of 5 years for all work completed by TruBuiit Construction. Inc only
17 Workmanship warranty does not cover damage to roofs caused by lightning, gale force (50 mph plus) winds.
tree fall. hurricane, tornado, hailstorm. impact of foreign objects. or other violent storm casualty, or damage
to roofs due to settlement, distortion, failure, or cracking of the roof deck, wails or foundation of building
or other structural components. Warranty applies to roof oniv
18 All excess materials remain the property of contractor(s) performing the work unless supplied by owner(s)
19 Homeowner acknowledges that contractor(s) is entitled to overhead and profit.. up to 26°x. on all trades
if secured as Dart of an insurance claim
20 Homeowner is responsible for HOA and Covenant compliance in all cases including but not limited to
color and material selections.
21 Homeowner is responsible for securing all valuables. hanging artwork. and any and all other items

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped
off to the Permit Desk at the City of Wheat Ridge. The report MUST BE SIGNED by the

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those

For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

12 This agreement shall expire one year from the contract date unless extended in writing by the project
manager, contractor(s), or if the Insurance company's approval is still penaing

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
