Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

This agreement (the 'Contract') is between Adams Roofing ('Company') and the customer(s) identified on the front side of this Agreement ('Customer) The terms and conditions contained on
the front side of this Contract are expressly made part of hereof and Company and Customer hereby agree upon and to be bound by the following additional terms and conditions: and will be
subject to all appropriate laws, regulations and ordinances of the State of Colorado and the local jurisdiction and the following terms and conditions.
MECHANIC'S LIEN NOTICE: We are required by law to provide Customer with this NOTICE.
(A) ANY PERSON OR COMPANY SUPPLYING LABOR OR MATERIALS FOR THIS IMPROVEMENT TO YOUR PROPERTY MAY FILE A LIEN AGAINST YOUR PROPERTY IF THAT
PERSON OR COMPANY IS NOT PAID FOR THE CONTRIBUTIONS.
(B) UNDER COLORADO LAW, YOU HAVE THE RIGHT TO PAY PERSONS WHO SUPPLIED LABOR OR MATERIALS FOR THIS IMPROVEMENT DIRECTLY AND DEDUCT THIS
AMOUNT FROM OUR CONTRACT PRICE, OR WITHHOLD THE AMOUNTS DUE FROM US UNTIL 120 DAYS AFTER COMPLETION OF THE IMPROVEMENT UNLESS WE GIVE
YOU A LIEN WAIVER SIGNED BY PERSONS WHO SUPPLIED ANY LABOR OR MATERIAL FOR THE IMPROVEMENTS INITIALS
AND WHO GAVE YOU TIMELY NOTICE.
1. This Contract is binding upon the Customer and Company upon execution, subject only to: (i) Approval of the Contract by Company's credit department and authorized officer, and (ii)
Approval by Customer's Insurance Company.
2. In the event of any default made by Customer in the payment of this Contract, Customer agrees that interest shall be added on the unpaid balance from the date of default AT A RATE OF ONE
AND ONE HALF (1-1/2) PERCENT PER MONTH (18% PER ANNUM) or the maximum amount allowed by applicable law.
Customer agrees and promises to pay to Company all reasonable and documented costs and expenses, including reasonable attorney's fees, paid or incurred or to be paid or incurred by
Company in collecting the sums due and owing under this Contract whether or not legal suit is filed with respect thereto and whether or not such costs and expenses are paid or incurred,
prior to or after entry of judgment.
3. The Company shall have no responsibility for damages arising out of or related to rain, fire, tornado, windstorm, or other perils, or acts of God, as are normally contemplated to be covered by
HOMEOWNER'S RISK INSURANCE or BUSINESS RISK INSURANCE, unless specifically agreed to by Company prior to commencement of work.
4. The quotation of price and terms by the Company maintained herein, does not include expenses or charges for bond insurance premiums or costs beyond normal insurance coverage, and
any such additional expenses, premiums or costs shall be added to the amount of the Contract.
5. Replacement, of deteriorated decking, fascia boards, roof jackets, ventilators, flashing or any other materials, unless otherwise expressly stated in this Contract, are not included in the price
and terms herein and will be charged as additional expenses on a time and material basis.
6. The Company reserves the right to revoke this proposal at any time prior to 90 days from date of acceptance by Company. After 90 days, the Company reserves the right to revise its price and
terms contained herein in accordance with costs in effect at that time.
7. The Company is not responsible for ice dams (thawing and freezing of ice, water or snow) or any other damage on or below the roof line due to leaks by excessive winds of 50 mph or greater.
8. Any and all warranties, guarantees or representations made by the Company hereunder are non -transferable by Customer.
9. The Customer may cancel this Contract by notifying the Company in writing within 3 days of the Customer's execution of the Contract. If the Contract is canceled by the Customer at any time
after 3 days from the date of Customer's execution, Customer shall pay to Company an amount equal to twenty five (25%) of the Contract price as liquidated damages, not as a penalty, and
the Company agrees to accept such payment as a reasonable and just compensation for Customer's cancellation of the Contract.
10. Notwithstanding Section 9 hereof, if material has to be reordered or restocked by Company due to cancellation of the Contract by the Customer, Customer shall be responsible for and pay a
restocking fee equal to 25% of the total Contract price.
11. If the Customer cancels the Contract after the work has begun by the company, the Customer shall pay the Company the full value of the work done as of the date of cancellation, plus the
total material costs incurred by the Company.
12. All direct costs, overhead and profit and / or supplement amounts paid to the Customer by the insurance provider are part of the negotiated amount between the Company and the insurance
provider, and are part of the Contract.
13. Any upgrade or additional work requested by the Customer that is not approved by the insurance provider will be the Customer's financial responsibility and will become part of the Contract.
14. The Customer's home will become the construction site. The Company will make reasonable attempts to work around existing landscaping or fixtures, but will not be responsible for any
damage done to landscaping or fixtures, including trees, bushes, shrubs, flowers, decorative fixtures during the construction process.
15. During the duration of the work performed, the Company will take all reasonable efforts to protect the Customer's property from further damage. Company shall have no responsibility for
interior damage and Customer or Customer's insurance provider shall be responsible for any interior damage that may have occurred or may occur.
16. The Company is not responsible for pre-existing construction deficiencies that manifest themselves during or after the construction process, including but not limited to, nail or screw pops in
drywall, wood rot, decking deflection, cracked driveways, or other defects due to poor installation or deterioration.
17. To the fullest extent permitted by law, and notwithstanding any other provision of this Agreement, the total liability of Company to Customer and anyone claiming by, through, or under
Customer for any and all claims, losses, costs, or damages whatsoever arising out of, resulting from or in any way related to the Contract or the work performed by Company, from any cause
or causes, including but not limited to tort, negligence, strict liability, breach of contract, or breach of warranty expressed or implied by Company, shall not exceed the total compensation
received by Company under this Contract.
18. This contract is composed of the front and backsides of this document and shall be considered the entire agreement.
19. The Company has the right to order excess materials. These materials will not be charged above the agreed upon Contract price and terms. All excess materials shall be the sole and
exclusive property of the Company.
20. Company is not responsible for and undertakes no duty, expressed or implied, to evaluating the Customer's home venting and circulation system status. Company will replace or install vents
and soffit according to Customer's work order.
21. The Company is not responsible for the realignment of satellite dish and will take every reasonable effort to reinstall it in the same or similar condition. Customer bears full responsibility for
re -aligning, unless agreed otherwise in writing by Company.
22. This Agreement contains the entire Agreement between Company and Customer and supersedes all previous and contemporaneous oral negotiations, commitments, writings and
understandings between the parties concerning the matters in this Contract. ANY REPRESENTATIONS, STATEMENTS, OR OTHER COMMUNICATIONS, NOT WRITTEN ON THIS CONTRACT ARE
AGREED TO BE IMMATERIAL, and not relied on by either party, and do not survive the execution of this Contract.
23. The laws of Colorado will govern the validity, construction, and performance of this Contract. Any legal proceedings related to this Contract will be brought in the State of Colorado, and both
the Company and the Customer consent and agree to exclusive jurisdiction of that court for this purpose.
24. Wherever possible, each provision of the Contract will be interpreted so that it is valid under the applicable law. If any provision of this Contract is to any extent invalid under the applicable
law, that provision will still be effective to the extent it remains valid. The remainder of the Contract also will continue to be valid, and the entire Contract will continue to be valid in other
jurisdictions.
