Dear Mr. DeLange, I have completed the third review of the above referenced document for the Exempla Lutheran Hospice received on March 30, 2005 and have the following comments: Final Plat Sheet 1: Please remove the words "LANGUAGE" from both easement paragraphs, and change the word STANDARD to UTILITY per the redlined comments. All other comments have been addressed. After the above-mentioned comments are addressed the Plat will be approvable from a Public Works standpoint. Sheet 2: 1. All previous comments have been addressed for Sheet 2. Please provide 2 copies of the revised Final Plat with the next submittal for approval. NPDES Permit The Federal Clean Water Act implemented by the EPA requires that stormwater discharges from certain types of facilities as part National Pollutant Discharge Elimination System (NPDES) be authorized under stormwater discharge permits. Construction sites greater than one (1) acre in size require an NPDES construction activities permit. The proposed area of disturbance is about 5 acres and shall therefore comply with NPDES regulations. The NPDES permit for construction activities is issued by the State through the Colorado Discharge Permit System (CDPS), and may be obtained by contacting the Colorado Department of Public Health & Environment, Water Quality Control Division. A copy of the CDPS Permit is to be submitted to the Department of Public Works prior to issuance of the Minor Grading/Fill and Building Permits. Application for Minor Grading/Fill Permit Prior to the commencement of any onsite grading, an Application for Grading/Fill Permit, along with the fees due will need to be submitted for review and approval. This Permit is generally issued concurrently with the Building Permit. Right-of-Way Construction Permit(s)/Licensing Prior to construction of any public improvements or utility service connections lying within the public right-of-way, the necessary Right-of-Way construction permit(s) and respective licensing will need to be submitted for processing by the City. Right-of-Way Construction Permits are issued only after approval of all required technical documents, including but not limited to, the Final Drainage Report & Plan, Final Plat, Final Development Plan, Traffic Impact Study, Storm Sewer Plans, Street Construction Plans, Grading & Erosion Control Plan, and easement or ROW dedications. All construction work performed under the Right-of-Way Construction Permit shall be warranted against defects for a period of 2 years per the City of Wheat Ridge Permit Testing and Inspection Requirements. Dear Mr. DeLange, I have completed the second review of the above referenced document for the Exempla Lutheran Hospice received on March 18, 2005 and have the following comments: Final Plat Sheet 1: The Legal Description does not match the graphical portion of the plat. The easterly line of the area of consolidation area (S00°04'03"E, 417.40' has been omitted in the current Legal. 2. Please correct the Legal Description per the redlined comments. 3. The vicinity map is too dark to be legible. 4. The subtitle needs to also be changed to reflect the platted area (NW '/4 of Section 26). 5. The revisions should be shown in the revisions table. 6. The Stormwater Detention Language needs to include the description of what is shown: "THE STORM DETENTION AREA HERE SHOWNASA STORMWA TER DETENTION POND SHALL BE CONSTRUCTED AND MAINTAINED..." The vicinity map does not coincide with the revised platted area. 3. The title needs to also be changed to the reflect the revised platted area. 4. Need to include the City's standard language for stormwater detention areas (please see attached sheet). Sheet 2: 1. Need to show all easements: a. Stormwater Detention Pond. b. Cross-Access (as indicated on Sheet 1). c. Parking & Ingress/Egress. d. Storm Sewer (Drainage Easement). e. Fire Apparatus Access Easement (or ROW). 2. Need to include the zoning for the parcels being consolidated. 3. The title needs to be changed to reflect the revised platted area. The Public Works Department requires Final Plat. General: A. Per our recent conversations, the Final Plat may be reviewed and ultimately approved administratively as a two-lot consolidation plat. The Code of Laws dictates that two-lot consolidation plats do not require a review by either Planning Commission or City Council. B. As mentioned in my previous memo, the additional parking, access and landscaping on the larger hospital campus will require an amendment to the existing Exempla Lutheran Medical Center Final Development Plan. The amendment process must be initiated prior to issuance of the building permit for the hospice. C. A Planning Commission hearing has been scheduled for Thursday, February 17, 2005. Public hearing notification signs must be posted by Thursday February 3, 2005. These signs will be prepared by the Community Development Department. 2. FDP Sheet One: A. Please amend line 4 in the Owner's Certification to read: "...does not create a vested property right." B. Please amend the City Council Approval to read: "Approved this day of 20_, by the Wheat Ridge City Council. " C. Under Character Development Standards, please amend the Required Fencing column to read: "...requirements of Section 26-603 of the Wheat Ridge Code of Laws." D.
