As a result of this update, the building permit submittal process has been updated. Please read this carefully as incomplete submittals will not be accepted. All features and fixtures shall be cleaned before final Client sign -off. > INTERRUPTION OF WORK Make West shall minimize interference with normal functioning of building and occupants and shall not impede emergency building evacuation with construction, equipment, materials, and procedures at building entrances and exits. Make West shall not interrupt mechanical or electrical services, shut down of building systems, services, and utilities without prior Client approval. > WALL PREPARATION All walls receiving vinyl graphic or wallcovering features shall require Client preparation to level 5 finish. Painted walls shall require a minimum cure time of 72 hours prior to installation. > MATERIALS All materials shall be new, unused, and of the highest quality in every respect, unless otherwise specified. Manufactured materials and equipment shall be installed as per manufacturer's recommendations and instructions. > ELECTRICAL Electrical work shall bear the label of Underwriters Laboratories, inc. (UL) approval as required. Make West shall verify power at each illuminated sign panel location and provide step down transformer where necessary to lamp fixtures. >ANCHORAGE Make West shall provide appropriate anchorage where required in order to provide for installation of all scheduled products. > FIXING & PATCHING All openings, voids, or unfinished surfaces created by removal or alteration of existing work, shall be closed or patched and finished as necessary to match existing continuous surfaces or new finishes required. Repairs shall be completed by Make West or by GC -supplied contractor in coordination with Client. > PUNCH LIST Upon completion of the work by Make West, the Client shall attend a punch list walk-through conducted by Make West. Make West shall take notes and prepare a list of final punch items to be completed or corrected. Punch list work shall be completed in a timely manner following the walk-through. The Client may withhold 10% of final project payment until punch list work is complete. >TOLERANCES All work shall be considered complete and may not be rejected provided the finished work falls within standard industry tolerances for the specified material and conditions. > DEFECTIVE WORK Non-compliant defective workmanship or quality revealed within the time required by guarantees shall be replaced by work conforming to the intent of the contract. No payment, either partial or final, shall be construed as an acceptance of defective work or improper materials. Make West shall warrant that all merchandise is free of defects in material or workmanship and shall provide Client with a warranty to replace or repair any merchandise that fails to conform to above for a period of no less than one (1) year from date of final acceptance. UNENFORCEABLE WORK Should any portion of the contract documents prove to be, for whatever reasons, unenforceable, such unenforceability shall not extend to the remainder of the contract nor shall it void any other provisions of the contract. CANCELLATION Both parties understand that Client or Designer may terminate the service at any time if, for any reason, the relationship is deemed unsatisfactory by either party. Upon written or verbal cancellation, Client is responsible for payment for all expenses incurred and any work done towards the completion of the project based on the percentage of the project completed that is determined by Designer. Should Client cancel the project following its completion, Client is responsible for full payment as per the agreed upon estimate plus all expenses incurred. In the event of cancellation, Designer retains ownership of all copyrights and original work created. CONFIDENTIALITY Make West acknowledges that he may receive or have access to information which relates to the Client's past, present, or future products, vendor lists, creative works, marketing strategies, pending projects/proposals, and other proprietary information. (MHES) to write a project design for the remediation plan per the Scope of Work (SOW) provided by MHES. The remediation project site is a residential building located at 10021 W. 26th Avenue, Wheat Ridge, CO (see below). The Colorado Department of Public Health and Environment's (CDPHE) Regulation 8, Part B defines an ACM as a material containing more than 1% asbestos. The intent of this project is to remove the identified ACM to be impacted prior to the renovation/demolition of the building. The following asbestos containing materials are included in the project design: Phase 1 — Professional Building • Floor Tile and Mastic (600 sq. ft.) • Textured Drywall (1,500 sq. ft.) Phase 2 — Former Grocery Store • Floor Tile and Mastic (9,000 sq. ft.) • Textured Drywall (600 sq. ft.) The containment will be conducted in one (1) phase, the containment will consist of a full containment and will include installation of critical barriers and negative airflow to the area, setting up a decontamination unit, pre -cleaning of the entire work space, removal and disposal of the above -identified ACM, final cleaning of the containment area(s) and final clearance air monitoring. Project Deisgn Page 4 All asbestos -containing waste generated during the project shall be disposed of at an asbestos - permitted landfill. Containerizing and transport of wastes shall be in accordance with applicable regulations. It is the Contractor's responsibility to arrange for delivery and pickup of waste containers without impeding the flow of pedestrian and vehicular traffic. 2.0 SPECIAL CONDITIONS 2.1 Alternative Procedures and Variances The Contractor will make all mandatory notifications to Federal and State entities regulating his work as required by applicable rules, regulations, and standards. This includes, but is not limited, to the National Emission Standards for Hazardous Air Pollutants (NESHAP) notification and abatement permit application provided to the Colorado Department of Public Health and Environment. There are no variances associated with this project. 2.2 Facility Occupancy Status The containment area will remain unoccupied for duration of the asbestos abatement project, until final -clearance criteria has been met. 2.3 Site Security Access to the work areas shall be strictly regulated. Entry to the regulated area is limited to asbestos -trained personnel and will be by the Supervisor's permission only. The perimeter of the work area should be monitored at all times during abatement by a competent person employed by the Contractor. Only asbestos -licensed personnel employed by the Contractor, Industrial Hygienist(s), or federal or state regulatory agency personnel will be granted access to the regulated work area. 2.4 Field Changes Minor modifications to this Project Design may be made only by CDPHE employees or by E3. Minor changes may include, but are not limited to, the relocation of decontamination facility and waste load -out, repositioning of negative air machines, changing view -port locations, etc. Project Deisgn Page I5 3.0 PROJECT PROTOCOLS 3.1 Standards and Primacy of Rules The following standards will be adopted as they pertain to asbestos abatement. In any instance where adopted standards are in conflict with each other, the most stringent shall apply. • CDPHE Regulation No. 8 Part B — Asbestos; • 29 CFR 1926.110 1, the OSHA Construction Industry Standard; • 40 CFR 61 Subpart M, EPA's NESHAP Asbestos Standard; • NIOSH/OSHA/USCG/EPA — Occupational: Safety & Health Guidance Manual for Hazardous Waste Site Activities, Sections 8-20; "Heat Stress and Other Physiological Factors"; and • All other applicable laws, rules and regulations, including but not limited to those relating to: o Workers' Compensation Insurance; o Liability Insurance; o All contract specifications and documents. 3.2 Work Included Work completion includes preparation of the work area, pre -cleaning activities, non -asbestos debris removal, removal of all listed ACM and packaging for disposal, final cleaning of the work area, final visual inspections, lockdown and final clearance monitoring. 3.3 Type and Location of ACM The Contractor is responsible for quantifying accurate amounts of ACM to be reported on the CDPHE Notification form and for disposal at an approved facility. 3.4 Site Access The contractor will have access to the building for the purpose of abatement, between 6 AM and 10 PM, 7 -days a week until the job is completed or Owner denies access. 3.5 Equipment Staging Area Contractor shall schedule pickup and delivery of all equipment, supplies and waste -transportation vehicles (roll -off dumpsters, trucks & trailers, etc.). Equipment may be staged adjacent to the work area, but only for the duration of the project. Project Deisgn Page 16 3.6 Utilities Service Electrical power and water supply will be supplied by the building owner to the Contractor. 3.7 Decontamination Facilities Project phases 1 and 2 will have 3 -stage decon and 2 stage waste-loadouts. The decon facility will be constructed and prepared in accordance with CDPHE Regulation 8 Part B. Section III.K. — "Decontamination Unit." The decontamination area must be maintained in a clean and orderly condition throughout the duration of the project. 3.8 Waste Load -Out Facilities Load -out and disposal procedures shall be in accordance with Division Approved Direct Waste Load -out System and also via the 2 -chamber waste load -out. Direct waste loadouts will be directly into pre -fabricated roll -off liners. 3.9 Critical Barriers All penetrations of the structure will be sealed with critical barriers. Openings of the HVAC ductwork will be sealed with critical barriers as well. All Critical barriers will adhere to CDPHE Regulation 8 Part B Section III.I — "Critical Barrier Installation." 3.10 Negative Pressure Ventilation The work area must be placed under negative air pressure and shall maintain a minimum pressure differential of -0.025 inches of water per the SOW. The negative air -pressure machines (NAM) are to be vented to the exterior of the containment. Air exchanges within the containment must occur at least 4 times per hour. Makeup air may be required for the containment areas. The volume each containment is provided below. The containment layouts are provided in Attachment A. Phase 1: Professional Building (total floor space is 3,500 sq. ft.) Air Exchange Calculations for Containment: Work Area: 35,000 ft3 x 4 per hr = [140,000 ft3/hr] * [1 hr/ 60 min] = 2,333 ft3/min [2,333 ft'/min] / [1,500 ft3/min] = 1.55 => 2 NAM As a conservative measure, the contractor will keep 1 NAM as a spare in case of an emergency or for difficulties in maintaining negative pressure. Project Deisgn Page 17 Phase 2: Former Grocery Store (total floor space is 12,000 sq. ft.) Air Exchange Calculations for Containment: Work Area: 120,000 ft3 x 4 per hr = [480,000 ft3/hr] * [I hr/ 60 min] = 8,000 ft3/min 8,000 ft3/min] / [1,500 ft3/min] = 5.33 => 6 NAM As a conservative measure, the contractor will keep 1 NAM as a spare in case of an emergency or for difficulties in maintaining negative pressure. 3.11 Containment Construction Containments for the asbestos removal shall be constructed pursuant to section III.N of CDPHE Reg. 8 — "Containment Components". 3.12 Pre -Cleaning Activities Pre -cleaning activities will be performed in accordance with CDPHE Regulation 8 Part B — Section III.L. Pre -cleaning of all surfaces will be accomplished after the regulated area is established. All workers performing pre -cleaning must don personal protective equipment (PPE). HEPA-equipped vacuums and wet methods shall be utilized for pre -cleaning activities. Any preparation activities occurring within arms -reach of ACM must be performed by workers utilizing PPE. 3.13 Asbestos Abatement/Removal Following the Environmental Consultant approval of the work -area and pre -cleaning preparations, the contractor may begin asbestos abatement activities using wet methods. Removal of ACM will be in accordance with CDPHE Reg. 8. ACM waste shall be placed in an enclosed, lined asbestos dumpster and/or in properly labeled waste disposal bags as the material is removed. 3.14 Asbestos Waste Load -Out All ACM waste must be double -bagged in minimum 6 -mil polyethylene bags or placed in an enclosed dumpster lined with a pre -fabricated "burrito" bag, and marked with the appropriate OSHA warning label for asbestos. All waste must also bear a generator label as required by EPA's 40 CFR 61 - Subpart M, NESHAP Standard. The Contractor will arrange for proper disposal of all waste materials. Contractor employees are required to utilize protective coveralls and respirators when handling ACM waste that is double - bagged. Project Deisgn Page 18 No asbestos -containing waste materials generated by abatement operations are to be stored within the containment. 3.15 Final Visual Inspection Criteria All interior surfaces of the work areas will be free of visible dust and debris. The containment must pass a final visual inspection by the Colorado -certified Industrial Hygienist/Air Monitoring Specialist (IH/AMS). Following acceptance of visual inspections, the IH/AMS will allow the contractor to remove the protective layers of polyethylene sheeting, leaving only critical barriers in place. 3.16 Encapsulation Encapsulation will not be used for the project. 3.17 Final Air -Clearance Monitoring Sample analysis for final clearance air monitoring for the containment areas will be performed by polarized light microscopy (PCM). A minimum five (5) samples will be aggressively collected from within the containment, with only critical barriers in place along with two (2) blanks per containment. Following formal notification from IH/AMS that fiber -concentration levels within the containment(s) are in accordance with Section III.P.3 "Final Clearance Air Monitoring and Sample Analyses," (less than or equal to 0.010 fibers per cubic centimeter) the AMS will instruct the Contractor to remove the critical barriers and take down the containment. The clearances will include verification that the dumpsters are detached from the containments. It may be necessary to clear the work area by Transmission Electron Microscopy (TEM) final clearance air sampling. The decision will be made by the Owner and/or Owners Representative after first evaluating the PCM final clearance samples from the containment. 3.18 Personal Air Monitoring Air samples will be collected by drawing air through a 25 -millimeter mixed cellulose ester filter, 0.8 um MCE filter 0.45 micron pore size, flexible tubing using constant flow low-volume personal sampling pumps. Each low -flow pump will be fitted with a computer microchip, which electronically regulates airflow and allows a fixed flow rate of air to pass through the filter. The flow rate and the volume of the air passed through the filter will be determined based on the National Institute for Occupational Safety and Health (NIOSH) 7400 analytical method. Each pump will be calibrated before and after the collection of each sample using a calibrated rotameter. The contractor shall be responsible for conducting personnel air -monitoring as applicable in accordance with OSHA 29 CFR 1926.1101 Asbestos Construction Standard. Results will be posted at the jobsite or made available to employees within 24 -hours. Project Deisgn Page 19 3.19 Electrical Hazards Control All electrical power utilized during the project will be on ground fault circuit interrupters (GFCI). All power sources must be located outside the work area. 3.20 Emergency Egress and Fire Protection Egress Points - The abatement contractor shall abide by the emergency egress rules and routes for the building. All contractor personnel shall receive emergency -procedure orientation specific to the building prior to initiation of abatement activities. Fire Protection Plan ✓ No items capable of initiating or sustaining combustion (lighters, matches, torches, etc.) will be allowed inside the containment. ✓ The use of flammable liquids or gasses is not permitted. Any electricity utilized must be on Ground Fault Circuit Interrupters (GFCI). ✓ A minimum of two, 2A:IOB:C rated fire extinguishers will be maintained on-site. There must be available at least one 2A:IOB: C rated fire extinguisher within a maximum -travel distance of 50 feet from any point in the work area. Workers will be trained in the use of fire extinguishers, emergency -evacuation plans, fire safety, and emergency reporting procedures prior to beginning work. ✓ All emergency exits will be labeled as such with tools available for breaching poly sheeting and keys will remain in door locks where necessary. ✓ The Contractor must implement a written emergency action and fire prevention plan in accordance with OSHA 29 CFR 1910.38 – Employee Emergency Plans and Fire Prevention Plans. Work areas shall have— or be fitted with temporary— emergency egress lighting. 3.21 Respiratory Protection Contractor personnel shall provide half face negative pressure respirators with NIOSH approved HEPA filters during all pre -clean, removal and waste load out procedures. (MHES) to write a project design for the remediation plan per the Scope of Work (SOW) provided by MHES. The remediation project site is a residential building located at 10021 W. 26th Avenue, Wheat Ridge, CO (see below). The Colorado Department of Public Health and Environment's (CDPHE) Regulation 8, Part B defines an ACM as a material containing more than 1% asbestos. The intent of this project is to remove the identified ACM to be impacted prior to the renovation/demolition of the building. The following asbestos containing materials are included in the project design: Phase 1— Professional Building • Floor Tile and Mastic (600 sq. ft.) • Textured Drywall (1,500 sq. ft.) Phase 2 — Former Grocery Store • Floor Tile and Mastic (9,000 sq. ft.) • Textured Drywall (600 sq. ft.) The containment will be conducted in one (1) phase, the containment will consist of a full containment and will include installation of critical barriers and negative airflow to the area, setting up a decontamination unit, pre -cleaning of the entire work space, removal and disposal of the above -identified ACM, final cleaning of the containment area(s) and final clearance air monitoring. Project Deisgn Page 4 All asbestos -containing waste generated during the project shall be disposed of at an asbestos - permitted landfill. Containerizing and transport of wastes shall be in accordance with applicable regulations. It is the Contractor's responsibility to arrange for delivery and pickup of waste containers without impeding the flow of pedestrian and vehicular traffic. 2.0 SPECIAL CONDITIONS 2.1 Alternative Procedures and Variances The Contractor will make all mandatory notifications to Federal and State entities regulating his work as required by applicable rules, regulations, and standards. This includes, but is not limited, to the National Emission Standards for Hazardous Air Pollutants (NESHAP) notification and abatement permit application provided to the Colorado Department of Public Health and Environment. There are no variances associated with this project. 2.2 Facility Occupancy Status The containment area will remain unoccupied for duration of the asbestos abatement project, until final -clearance criteria has been met. 2.3 Site Security Access to the work areas shall be strictly regulated. Entry to the regulated area is limited to asbestos -trained personnel and will be by the Supervisor's permission only. The perimeter of the work area should be monitored at all times during abatement by a competent person employed by the Contractor. Only asbestos -licensed personnel employed by the Contractor, Industrial Hygienist(s), or federal or state regulatory agency personnel will be granted access to the regulated work area. 2.4 Field Changes Minor modifications to this Project Design may be made only by CDPHE employees or by E3. Minor changes may include, but are not limited to, the relocation of decontamination facility and waste load -out, repositioning of negative air machines, changing view -port locations, etc. Project Qeisgn Page I5 3.0 PROJECT PROTOCOLS 3.1 Standards and Primacy of Rules The following standards will be adopted as they pertain to asbestos abatement. In any instance where adopted standards are in conflict with each other, the most stringent shall apply. • CDPHE Regulation No. 8 Part B — Asbestos; • 29 CFR 1926.110 1, the OSHA Construction Industry Standard; • 40 CFR 61 Subpart M, EPA's NESHAP Asbestos Standard; • NIOSH/OSHA/USCG/EPA — Occupational: Safety &. Health Guidance Manual for Hazardous Waste Site Activities, Sections 8-20; "Heat Stress and Other Physiological Factors"; and • All other applicable laws, rules and regulations, including but not limited to those relating to: o Workers' Compensation Insurance; o Liability Insurance; o All contract specifications and documents. 3.2 Work Included Work completion includes preparation of the work area, pre -cleaning activities, non -asbestos debris removal, removal of all listed ACM and packaging for disposal, final cleaning of the work area, final visual inspections, lockdown and final clearance monitoring. 3.3 Type and Location of ACM The Contractor is responsible for quantifying accurate amounts of ACM to be reported on the CDPHE Notification form and for disposal at an approved facility. 3.4 Site Access The contractor will have access to the building for the purpose of abatement, between 6 AM and 10 PM, 7 -days a week until the job is completed or Owner denies access. 3.5 Equipment Staging Area Contractor shall schedule pickup and delivery of all equipment, supplies and waste -transportation vehicles (roll -off dumpsters, trucks & trailers, etc.). Equipment may be staged adjacent to the work area, but only for the duration of the project. Project Deisgn Page 16 3.6 Utilities Service Electrical power and water supply will be supplied by the building owner to the Contractor. 3.7 Decontamination Facilities Project phases 1 and 2 will have 3 -stage decon and 2 stage waste-loadouts. The decon facility will be constructed and prepared in accordance with CDPHE Regulation 8 Part B. Section III.K. — "Decontamination Unit." The decontamination area must be maintained in a clean and orderly condition throughout the duration of the project. 3.8 Waste Load -Out Facilities Load -out and disposal procedures shall be in accordance with Division Approved Direct Waste Load -out System and also via the 2 -chamber waste load -out. Direct waste loadouts will be directly into pre -fabricated roll -off liners. 3.9 Critical Barriers All penetrations of the structure will be sealed with critical barriers. Openings of the HVAC ductwork will be sealed with critical barriers as well. All Critical barriers will adhere to CDPHE Regulation 8 Part B Section III.I — "Critical Barrier Installation." 3.10 Negative Pressure Ventilation The work area must be placed under negative air pressure and shall maintain a minimum pressure differential of -0.025 inches of water per the SOW. The negative air -pressure machines (NAM) are to be vented to the exterior of the containment. Air exchanges within the containment must occur at least 4 times per hour. Makeup air may be required for the containment areas. The volume each containment is provided below. The containment layouts are provided in Attachment A. Phase 1: Professional Building (total floor space is 3,500 sq. ft.) Air Exchange Calculations for Containment: Work Area: 35,000 ft3 x 4 per hr = [140,000 ft'/hr] * [1 hr/ 60 min] = 2,333 ft'/min [2,333 ft'/min] / [1,500 ft3/min] = 1.55 => 2 NAM As a conservative measure, the contractor will keep 1 NAM as a spare in case of an emergency or for difficulties in maintaining negative pressure. Project Deisgn Page 17 Phase 2: Former Grocery Store (total floor space is 12,000 sq. ft.) Air Exchange Calculations for Containment: Work Area: 120,000 ft' x 4 per hr = [480,000 ft3/hr] * [1 hr/ 60 min] = 8,000 ft3/min 8,000 ft'/min] / [1,500 ft3/min] = 5.33 => 6 NAM As a conservative measure, the contractor will keep 1 NAM as a spare in case of an emergency or for difficulties in maintaining negative pressure. 3.11 Containment Construction Containments for the asbestos removal shall be constructed pursuant to section III.N of CDPHE Reg. 8 — "Containment Components". 3.12 Pre -Cleaning Activities Pre -cleaning activities will be performed in accordance with CDPHE Regulation 8 Part B — Section III.L. Pre -cleaning of all surfaces will be accomplished after the regulated area is established. All workers performing pre -cleaning must don personal protective equipment (PPE). HEPA-equipped vacuums and wet methods shall be utilized for pre -cleaning activities. Any preparation activities occurring within arms -reach of ACM must be performed by workers utilizing PPE. 3.13 Asbestos Abatement/Removal Following the Environmental Consultant approval of the work -area and pre -cleaning preparations, the contractor may begin asbestos abatement activities using wet methods. Removal of ACM will be in accordance with CDPHE Reg. 8. ACM waste shall be placed in an enclosed, lined asbestos dumpster and/or in properly labeled waste disposal bags as the material is removed. 3.14 Asbestos Waste Load -Out All ACM waste must be double -bagged in minimum 6 -mil polyethylene bags or placed in an enclosed dumpster lined with a pre -fabricated "burrito" bag, and marked with the appropriate OSHA warning label for asbestos. All waste must also bear a generator label as required by EPA's 40 CFR 61 - Subpart M, NESHAP Standard. The Contractor will arrange for proper disposal of all waste materials. Contractor employees are required to utilize protective coveralls and respirators when handling ACM waste that is double - bagged. Project Deisgn .... ___._.._._.....__._..... No asbestos -containing waste materials generated by abatement operations are to be stored within the containment. 3.15 Final Visual Inspection Criteria All interior surfaces of the work areas will be free of visible dust and debris. The containment must pass a final visual inspection by the Colorado -certified Industrial Hygienist/Air Monitoring Specialist (IH/AMS). Following acceptance of visual inspections, the IH/AMS will allow the contractor to remove the protective layers of polyethylene sheeting, leaving only critical barriers in place. 3.16 Encapsulation Encapsulation will not be used for the project. 3.17 Final Air -Clearance Monitoring Sample analysis for final clearance air monitoring for the containment areas will be performed by polarized light microscopy (PCM). A minimum five (5) samples will be aggressively collected from within the containment, with only critical barriers in place along with two (2) blanks per containment. Following formal notification from IH/AMS that fiber -concentration levels within the containment(s) are in accordance with Section III.P.3 "Final Clearance Air Monitoring and Sample Analyses," (less than or equal to 0.010 fibers per cubic centimeter) the AMS will instruct the Contractor to remove the critical barriers and take down the containment. The clearances will include verification that the dumpsters are detached from the containments. It may be necessary to clear the work area by Transmission Electron Microscopy (TEM) final clearance air sampling. The decision will be made by the Owner and/or Owners Representative after first evaluating the PCM final clearance samples from the containment. 3.18 Personal Air Monitoring Air samples will be collected by drawing air through a 25 -millimeter mixed cellulose ester filter, 0.8 um NICE filter 0.45 micron pore size, flexible tubing using constant flow low-volume personal sampling pumps. Each low -flow pump will be fitted with a computer microchip, which electronically regulates airflow and allows a fixed flow rate of air to pass through the filter. The flow rate and the volume of the air passed through the filter will be determined based on the National Institute for Occupational Safety and Health (NIOSH) 7400 analytical method. Each pump will be calibrated before and after the collection of each sample using a calibrated rotameter. The contractor shall be responsible for conducting personnel air -monitoring as applicable in accordance with OSHA 29 CFR 1926.1101 Asbestos Construction Standard. Results will be posted at the jobsite or made available to employees within 24 -hours. Project Deisgn Page 19 3.19 Electrical Hazards Control All electrical power utilized during the project will be on ground fault circuit interrupters (GFCI). All power sources must be located outside the work area. 3.20 Emergency Egress and Fire Protection Egress Points - The abatement contractor shall abide by the emergency egress rules and routes for the building. All contractor personnel shall receive emergency -procedure orientation specific to the building prior to initiation of abatement activities. Fire Protection Plan ✓ No items capable of initiating or sustaining combustion (lighters, matches, torches, etc.) will be allowed inside the containment. ✓ The use of flammable liquids or gasses is not permitted. Any electricity utilized must be on Ground Fault Circuit Interrupters (GFCI). ✓ A minimum of two, 2A:IOB: C rated fire extinguishers will be maintained on-site. There must be available at least one 2A:IOB: C rated fire extinguisher within a maximum -travel distance of 50 feet from any point in the work area. Workers will be trained in the use of fire extinguishers, emergency -evacuation plans, fire safety, and emergency reporting procedures prior to beginning work. ✓ All emergency exits will be labeled as such with tools available for breaching poly sheeting and keys will remain in door locks where necessary. ✓ The Contractor must implement a written emergency action and fire prevention plan in accordance with OSHA 29 CFR 1910.38 – Employee Emergency Plans and Fire Prevention Plans. Work areas shall have— or be fitted with temporary— emergency egress lighting. 3.21 Respiratory Protection Contractor personnel shall provide half face negative pressure respirators with NIOSH approved HEPA filters during all pre -clean, removal and waste load out procedures. Contractor shall provide proof of medical fitness to wear respiratory protection and current fit testing documentation for all employees.
