Dear Mr. Krombein, I have completed my review of the second submittal of the Drainage Letter received on May 6, 2009 and the third submittal of the Civil Construction Plans received on May 8, 2009. This letter is to inform you that all of the above-mentioned documents for this development have been reviewed and are hereby approved for construction by the Department of Public Works. Please be aware of the following items regarding the construction of the project: Public Improvement Restoration/Debris Tracking It will be the responsibility of the of the contractor for the project to repair any damage to the existing public improvements along the Youngfreld Service Road as a result of related construction traffic in the area. Also, the contractor will be responsible for maintaining the Youngfield Service Road on a regular basis such that it remains free of construction debris and tracking from construction traffic accessing the site. 2. Drainage Certification Required Prior to C.O. Upon completion of the drainage improvements, the Engineer-of-Record Mr. Brian Krombein, P.E. shall provide to the City of Wheat Ridge a written, signed and sealed Drainage Certification Letter stating that the site was completed per the approved Final Grading & Erosion Control Plans, that all drainage facilities were constructed and shall function as defined in the approved Drainage Letter, and that the site has been accurately surveyed to confirm that the construction of all drainage-related items were completed in accordance with these documents. The Drainage Certification Letter shall be submitted to the City for review and approval, and shall be accompanied with As-Built Plans for all constructed drainage facilities prior to issuance of a Certificate of Occupancy (or Certificate of Completion). Two (2) copies of the "As-Built" Plans shall be submitted as follows: a. 1 copy shall be on full-size 24" X 36 " bond paper, signed and sealed, and b. 1 copy shall be on electronic media (CD-ROM) in AutoCAD 2000-2007.dwg format only. www.ci.wheatridge.co.us Public Works Engineering May 11, 2009 Page 2 3. Right-of-Way Construction Permit(s), Licensing, and Bonding Prior to any construction within the public right-of-way, the necessary Right-of-Way Construction Permit(s) and respective licensing will need to be submitted for processing by the City. Right-of-Way Construction Permits are issued only after approval of all required technical documents, including but not limited to, the Final Drainage Report & Plan, Final Plat, Final Development Plan, Traffic Impact Study, Storm Sewer Plans, Street Construction Plans, Grading & Erosion Control Plan, Traffic Control Plan, and easement or ROW dedications. In addition, if the cost of the proposed public improvements exceeds $10,000, additional warranty bonding shall be required. 4. 2-Year Warranty Period for Public Improvements The contractor shall warrant any work performed within the public Right-of-Way for a period of two (2) years, commencing upon completion of all constructed public improvements and Final Acceptance by the City Inspector. Prior to the expiration of said 2- year Warranty Period, if the City Inspector at any time deems that the constructed public improvements are in such condition as to require replacement or repairs, the contractor shall complete such repair work upon request per the City of Wheat Ridge Permit Testing and Inspection Requirements. 5. Materials Testing for any Construction within the Public Right-of-Way It will be the responsibility of the developer/owner, to provide the necessary testing, as applicable for the scope of this project, for sub-grade compaction and other related material tests for any improvements to be constructed within the public Right-of-Way. Unless otherwise designated, all referenced specifications, standards or policies shall be the latest edition as revised or updated by approved supplements published and issued prior to the date of the building permit. REQUIREMENTS All materials sampling and testing shall be performed by certified, experienced and qualified materials testing technicians who work under the supervision of a registered professional engineer in the State of Colorado. All materials sampling and testing equipment shall be serviceable and have current calibrated certifications. Soil classifications and moisture-density curves shall be provided to the Department of Public Works inspector prior to in-place density testing. Materials testing technicians shall furnish copies of failed test results to the inspector as promptly as the results become, available. On a weekly basis when testing is being performed, the developer shall furnish the inspector with copies of all test results taken during the prior week and a cover letter, signed by the supervising registered professional engineer, which summarizes the results and discusses any failed tests or inconsistencies. The City materials testing requirements are provided in Table 720-1. All testing procedures of verification and central lab requirements shall be as specified in the Frequency Guide Schedule of the Colorado Department of Transportation Field Materials Manual. One test is required for any fraction of the specified frequency. I have reviewed proposed plan set and have the following comments: FRONT SHEET The site plan does not match that approved for the Final Development Plan (FDP). All three different sets of plan documents (building permit submittal, FDP and civil plan set) should be consistent. The majority of the following comments are a result of these differences. 1. In the northeast corner of the site, please specify the type of ground cover or material proposed. 2. On the graphic of the existing restaurant, specify that the total square footage of 4,162 square feet is as amended and approved by this building pen-nit. 3. Add a site data breakdown showing the total amount of the following categories: building coverage, landscaped coverage and hard surfaced coverage. Show categories in square footage and percent of total site. 4. A landscape schedule showing existing and proposed plan materials should be added. 5. At the southwest corner of the site, access to the west into Lot 4 is not allowed. A physical barrier such as a concrete curb will be required to be installed at this location. There are inadequate street trees planted along the northern property line adjacent to Youngfield Service Road. Section 26-502.D.3. of the zoning and development code requires there be one street tree for every 30' of street frontage for commercial properties. Using this figure and subtracting curb cuts, the required number of street trees is seven. Any existing trees located in the area between the northern building wall and street can be counted toward this number. 6. Modify the western curb cut island to Youngfield Service Road to accurately reflect the sidewalk which is displacing landscaping on the drawing. The landscape schedule and land use chart should reflect the change in landscaping quantities and percent of landscaping versus impervious surface. SHEET A2.1 7. All signage must be by separate permit. 8. Is any new exterior lighting being proposed? If so, a photometric sheet should be added including informationregarding existing and proposed fixtures.
