As requested, we performed onsite observation of the remedial repairs and enhancements
to the roof including the installation of new roof support beams and rafter reinforcement, at
the address referenced above. Our observations were performed during a portion of the
installation process.
The scope of work performed consisted of observation of portions of the remedial work and
placement of various structural components, during installation. The purpose of my review
was to confirm the extent and location of the remedial work and to verify that the repairs
were installed in substantial compliance with the permit plans and generally accepted indus-
try standards, procedures and practices. The following items were reviewed subsequent to
installation:
• Installation of new attic beams and associated framing elements.
• Installation of new beam bracing.
• Installation of new roof rafter sister joists and associated framing.
• Installation of associated blocking.
It should be noted that the interior foundation support that was proposed to be added in the
crawispace was omitted due to the presence of an existing foundation and footing. This
existing footing was used to support the interior column support adjacent to the bathroom.
My observations were performed during the installation of portions of the above-mentioned
elements. Not all of the repairs were directly observed. In my opinion, these specific
portions of the remedial work appear to be installed in a consistent manner with that shown
on the permit plans, previously submitted. To the best of my professional knowledge, belief
and opinion, and based on limited observations, it appears that the remedial work currently

in place appears to, generally, meet the applicable Codes, industry standards and
regulations of agencies having jurisdiction.

On 2-9-21, 1 performed a limited inspection at the address listed above. I inspected for possible ACM in
the customers master bedroom wall, living room/dining room wall, and the attic insulation. The purpose
of this inspection is to remodel. No other part of the house was inspected.
1. Regulations
The EPA and the Colorado Department of Public Health and Environment (CDPHE) require all building
materials with Asbestos Containing Materials (ACM) contain less than 1% asbestos via PLM analysis. OSHA
requirements are more stringent.
It is required by law all friable and non -friable ACM be removed prior to renovation activities that may
disturb verifiable and assumed ACM. Furthermore, all friable ACM must be removed prior to demolition.
Point counting is required for bulk samples where the asbestos content is less than 10%. Federal and
State agencies define ACM as those containing more than 1%. National Emission Standards for Hazardous
Air Pollutants (NESHAP) requires materials with ACM less than 10% be point counted or be "assumed"
ACM. If point counting is done, the result supersedes the non -point counted PLM result.

Environmental matrices by the National Voluntary Laboratory Accreditation Program (NVLAP), Lab Code
101896-0 for Transmission Electron Microscopy (TEM) and Polarized Light Microscopy (PLM) analysis and the
American Industrial Hygiene Association (AIHA), Lab ID 101533 - Accreditation Certificate #480 for Phase

VERIFICATION PRIOR TO PROCEEDING WITH THE WORK RELATED TO THAT CONDITION. FIELD VERIFICATION MUST BE COMPLETED IN A TIMELY MANNER
AND COMMUNICATED BY THE CONTRACTOR TO ALL AFFECTED PARTIES IN ORDER TO AVOID DELAYS TO THE WORK.

REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE

Right of First Refusal
Owner grants Precision Homes, LLCRoofing Division an exclusive right to perform work and repair on any final estimate generated by the insur-
ance company(ies).
Cash Bids
All cash bids are valid for thirty (30) days due to quarterly fluctuations in material and or labor costs. Payment terms for "cash bid" jobs require
(50%) due before materials are delivered; remaining balance shall be immediately due and payable upon completion of the specified work.
Agent
Owner acknowledges that Precision Homes, LLC Roofing Division is appointed as Owner's agent and representative in working on their behalf to
assist in Owner's insurance claim negotiation. Owner agrees to cooperate with Precision Homes, LLC Roofing Division in this regard and shall
share any and all documents or correspondence received to Owner's insurance claim with Precision Homes, LLC Roofing Division Owner ap-
points Precision Homes, LLC Roofing Division and it's authorized representatives as it's attorney-in-fact to take any action necessary to effectu-
ate the terms of this contract.
Material Selection
Owner is solely responsible for the material and color selection of all products utilized by Precision Honies, LLC" Roofing Division and agrees to
insure that all selections shall meet or exceed any neighborhood, city, homeowners association, zoning, master lease, building code or any other
requirements. Owner shall indemnify, defend and hold Precision Llomes, LLC Roofing Division harmless from any action brought by any person,
group or entity concerning the quality, type, color or other characteristic of the material selected by the owner.
Acknowledgment of Work by Owner
Owner acknowledges that Precision Homes, LLC Roofing Division shall not be responsible for minor inconveniences and damages resulting from
Precision Haines, LLC Roofing Division work at the contract property in accordance with all generally accepted industry standards and practices
generally associated with this type of work.
Failure to Perform
Owner acknowledges that Precision Homes, LLC Roofing Division will not be responsible for any failure to perform it's obligation due to labor
controversies, strikes, acts of nature, material shortages, labor shortages, eminent domain actions, court or any other government agency or authori-
ty declarations, or any other circumstances beyond the control of Precision Homes, LLC Roofing Division
Additional Work
Costs for the replacement or repair of any roofing system components (including, but not limited to decking, fascia, swamp coolers, solar panels,
vents, exhaust, HVAC components, chimney or chimney flashing, step or headwall flashing, windows, etc.) or additional work not specifically
stated in this contract are not included. In addition, any work or damage uncovered during the repair or replacement process that was unforeseen is
not included in this contract. Such work will be billed separately as an extra and billed according to the fair market prices. Precision Homes, LLC
Roofing Division agrees not to make any changes in the schedule of work, design, or of the specifications without written authorization by the
Owner.
Access
Customer will provide required access to their property during daylight hours in order for Precision (fomes, LLC Roofing Division to perform the
work stated in this contract. Customer hereby agrees to pay a reasonable charge if access is either denied or unobtainable and arrangements have
not been previously made.
Limited Warranty
Precision Homes, LLC Roofing Division shall provide a limited warranty against leaks on total roof installations performed by Precision Homes,
LLC Roofing Division should such leaks result be a result of Precision Homes, LLC Roofing Division faulty installation. Such warranty does not
extend to leaks or water damage below the roof line or any damage resulting from lightning, snow, excessive wind, ice or hail during the warranty.
Owner hereby acknowledges that no other warranties, expressed or implied.
Permits and Codes
Precision Homes, LLC Roofing Division shall obtain all necessary building permits, and will comply with all building and code regulations and
ordinances whether or not covered by the specifications and drawings for the work.
Work Performance
Precision Homes, LLC Roofing Division shall perform all specified work in substantial compliance with all generally accepted industry standards
and agrees to keep the premises clean and orderly and will remove all debris.
Accounts
Accounts are past due if full payment is not received ten(10) days after work is substantially completed. In the event that the insurance company
has issued payment to anyone in connection with repair or replacement of the property, then full payment for any work performed is due. If the
customer then does not use Precision Homes, LLC Roofing Division then a 20% charge is immediately due and payable by owner to Precision
Homes, LLC Roofing Division The amount due is for time spent preparing, measuring, getting material, allocating resources by Precision Homes,
LLC Roofing Division and the parties agree 20% is reasonable estimate of loss. Customer acknowledges that Customer shall be responsible for all
costs of collection of any amounts due including, but not limited to, attorney's fees, expenses, costs and expert witness fees. All amounts past due
for more than 30 days will accrue interest at the rate of twenty-one percent (211/6) per annum.
Dispute Resolution
The parties agree that the provisions of the Colorado Construction Defect Action Reform Act, at C.R.S. 13-20-801, et. Seq. shall apply to any dis-
pute arising from this contract.
Cancellation
If this contract is cancelled by the Customer after the third business day and prior to the commencement of work, Customer shall pay the Precision
Homes, LLC Roofing Division (20%) of the contract price or as determined by the insurance loss sheet replacement cost value, for services. Preci-
sion .Homes, LLC Roofing Division agrees to accept (20%) as reasonable and just compensation for said cancellation. If this contract is cancelled
by the Customer after commencement of work, customer will pay in addition for materials, labor and associated profit for work completed up to
and including the date cancellation is received in writing by Precision Homes, LLC Roofing Division. Payment should be made to Precision
Homes, LLC Roofing Division and delivered or mailed to the address listed on the front of this contract.
