Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

5. Interpretation:
a. Interpretabon ofDocuments: The Residential Roofing Contract, drawings and specifications are intended to supplement one another. In the event of a conflict, the specifications shall control the drawings,
and the Residential Roofing Contract shall control both. Ifwork is displayed on the drawings but not called for in the specifiations, or ifthe work is called for in the specifications but not displayed on the
drawings, Roofing Contractor shall be required to perform the work as though Awere called for and displayed in both documents.
b. Entire Agreement: This Residential Roofing Contract constitutes then entire agreement of the parties. No other agreements, oral or written, pertaining to the work to be performed under this Residential
Roofing Contract exist between the parties. This Residential Roofing Contract may only be modified by a written agreement signed by all parties.
c Governing Law and Venue: This Residential Roofing Contract shall be interpreted and governed in accordance with the laws of the Stare of Colorado. The venue of any action to enforce this Residential Roofing

I responded to a voicernail message today left without a person's name. The man on the messaged ended up
being Frank Wilson, re: 3901 Miller Ct.
He spoke w/ you yesterday about work being done over the weekend (crawispace vents, sump pump, etc...), on
Saturday evening, without a permit. He has a concern about the vents going through asbestos siding. Duct work,
and the sump pump being installed w/o a permit. You stated to him that you were going to be posting the site
with a stop work order. It is currently 3:13 PM, and he has noticed that one has not been posted as of yet.
He wants copies of the stop work order and any building permits pulled for that address. I told him that any permit
we issue or document that we post is public record, and he can come in at any time to have copies made. He
wanted a phone call when the house was posted or permits issued, in which case I told him it would be easier for
him to call the building department and inquire if any permits had been pulled, etc... if so, he could ask for copies
to be made at that time, At this time — I have no building permits issued to that address since 1995.
Please follow up with this property, Frank and post the building as you stated yesterday,

and what will be required for the city?

Could you please call this gentleman (then let me know what you would like me to do)? He would like to speak to
Bill's supervisor.
Recv'd a phone call from Frank Wilson: 303-881-9199 today approximately 10:15 AK I was on the phone with
him for close to 20 minutes. He is to the point that he is going to take legal action against the owner of the
property, the property management company and the City of Wheat Ridge for their involvement/or lack of
involvement on his complaint. He has all the legal numbers ready to dial, and will and has no qualms about
smearing this all over the news (channel 4, 7, 9, etc). He is very upset. He has upper respiratory problems, sinus
problems and has developed a skin disease. Tests are pointing towards all the mold, bacteria & other pathogen
spores entering in from the "wet"crawlspace,
Regarding: 3901 Miller Ct.
Frank dropped off a rental complaint form on Friday, May 4, 2007. The complaint for was given to Cindy. On
5/7/07 we received, on the inspection line, a message from Mr. Wilson about having an inspector come out to the
house with the property management company, etc, (I am assuming Cindy gave him the inspection line number,
and is saying Bill should be doing this inspection.) I wrote Mr. Wilson into the inspection log for the 8ffi. I placed
his phone number and the number of the property managers on the log as well as to do the inspection between
10AM-2PM, I also forwarded the complaint message from the inspection to Bill's voicemail w/ a note for him to
listen to his voicemail,
He called again on 5/8/07 on the inspection line to set up an inspection. (I didn't know if Bill ever talked to him or
not for the inspection the previous day — so I forwarded that message to Bill's voicemail.)
