Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

2. This. pen -nit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

INSURANCE ALLOWANCE AGREEMENT:
I / We agree to retain Watershed Roofing and Restoration (contractor) to repair/replace the damaged or entire portion of the property described on this contract contingent upon the
contractor obtaining my insurance company's approval and payment to have said work completed. It is agreed that the contractor is empowered to contact my insurance carrier and to
meet with their representative in order to negotiate the amount of damage, the cost of the repair or replacement of the damage to the above property. Upon insurance carrier agreement
with the contractor and the down payment being made the work will be scheduled and may begin on a first in first out basis.
Checks shall be made payable to Watershed Roofing and Restoration.
In the event that the insurance carrier fails to negotiate in a fair and reasonable manner, the contractor shall not be held liable and this agreement shall be void. The contractor will
release the complete file to the Commissioner of Insurance at the customer's request. Insured out of pocket expense will not exceed the insured (purchaser) policy deductible for the
work agreed upon between the insurance carrier and the contractor. Insured also agrees to provide all the necessary documentation for the claim (including documentation to facili-
tate payment from insurance carrier and/or mortgage holder). The contractor reserves the right to file for supplemental claims due to material and/or labor increases due to a storm
environment or to insurances adjuster's mistakes, errors, or omissions.

I/We agree that by signing above we agree to all of the conditions on both the front and back of this agreement.
1. Watershed Roofing and Restoration shall hold in trust any payment from the property owner until the roofing contractor has
delivered materials or has performed a majority of the work. Watershed Roofing and Restoration has a 72 hour right of rescis-
sion. Watershed Roofing and Restoration will not conduct any work on your claim within the first 3 days of this contract. After the
72 hours have passed, this proposal will become a binding contract. This contract may also be rescinded within 72 hours after
the property owner receives written notice that its insurance claim has been denied. Existing Colorado statute related to property
insurance fraud (CRS 18.13.119.5) states that paying, waiving or rebating a property owner's insurance deductible is a Class 2
Misdemeanor.
2. If material has to be reordered or restocked because of a cancellation or change by the customer, there will be a restocking
fee equal to twenty percent (20%) of the contract price.
3. A copy of General Liability and Worker's Compensation insurance has been supplied to you in our property owner packet. If
you ever need a copy, please contact the office and we will be happy to provide you additional copies.
4. Watershed Roofing and Restoration agrees to perform the specified repairs/replacement in accordance with generally ac-
cepted roofing and construction guidelines, adhering to local codes in effect at the time.
5. Payment terms: 50% payment upon delivery of material and 50% upon completion. Payment is due within ten days of receipt
of the invoice. Completion is not contingent upon inspection by the applicable governmental entity where the work is performed.
Warranty on labor begins upon completion of the project and upon full payment of the contract. Interest charges of one percent
{1 %} per month will be charged on all past due balances and customer shall also pay all costs of collection including reason-
able attorney fees and costs. Watershed Roofing and Restoration reserves the right to record a mechanics lien on the property
if payment is not made in a timely manner. Unless otherwise required by law, the parties agree that the Courts for the County of
Jefferson, State of Colorado shall have exclusive jurisdiction over any legal actions. The parties agree and consent to the juris-
diction of that court over the subject matter of those controversies and over them personally.
6. Price of proposal is valid only for 30 days from proposal date based on quarterly material price increases.
7. The homeowner is responsible for getting Homeowners Association approval of roofing products and color, if necessary.
8. The property owner is responsible for facilitating mortgage process and cannot hold Watershed Roofing and Restoration li-
able in delays caused by the mortgage endorsement process.
9. Property owner acknowledges Watershed Roofing and Restoration as the general contractor and as such, Watershed Roof-
ing and Restoration will be entitled to 10% overhead and 10% profit.
10. Extra material is ordered on every job to avoid issues or complications, therefore all excess materials remain the property of
Watershed Roofing and Restoration.
11. The company shall not be liable for failure of performance due to labor controversies, strike, fire, hail, weather, inability to
obtain material from usual sources, or any other circumstances beyond the control of the company.
12. Warranty: Five (5) year workmanship warranty on roof replacement, excluding 90# mineral surface rolled roofing, and one
(1) year workmanship warranty for all other work. The company will provide a service call for roof inspection as long as the
customer is the current owner of the property and all obligations are paid in full. No warranty exists until this contract is paid in
full. Watershed Roofing and Restoration is not responsible for any leaks or water damage below the roof line, including but not
limited to leaks caused by excessive wind, ice or hail during the period of the warranty. The warranty does not cover events or
circumstances not the fault of Watershed Roofing and Restoration including, but not limited to, ordinary wear and tear, improper
maintenance and damage by others. The warranty for the materials is limited to the manufacturer's warranty and the Homeown-
er agrees to look to the manufacturer for any defects or deficiencies in the materials.
13. Client agrees to not hold Watershed Roofing and Restoration liable for the following damages that may occur during roofing
construction: slight scratching to the gutters, damage to plants and trees due to roofing and tear off, cracks in driveways, interior
damage, such as split crown molding, cracked plaster or sheet rock, nail pops, or falling dust or debris, or items knocked off the
wall or shelf due to the construction. Watershed Roofing and Restoration is not responsible for pre-existing damages to any part
of the property, which includes, but is not limited to solar panels, evaporative coolers or existing skylights.
14. The price on the contract does not include replacement of deteriorated decking, fascia boards, swamp coolers, ventila-
tors, and chimneys, counter flashing or any other work completed for the property owner not stated in this contract, and will be
charged as an extra on a time and material basis.

approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the

INSURANCE ALLOWANCE AGREEMENT:
I ! We agree to retain Watershed Roofing and Restoration (contractor) to repair/replace the damaged or entire portion of the property described on this contract contingent upon the
contractor obtaining my insurance company's approval and payment to have said work completed. It is agreed that the contractor is empowered to contact my insurance carrier and to
meet with their representative in order to negotiate the amount of damage, the cost of the repair or replacement of the damage to the above property. Upon insurance carrier agreement
with the contractor and the down payment being made the work will be scheduled and may begin on a first in first out basis.
Checks shall be made payable to Watershed Roofing and Restoration.
In the event that the insurance carrier fails to negotiate in a fair and reasonable manner, the contractor shall not be held liable and this agreement shall be void. The contractor will
release the complete file to the Commissioner of Insurance at the customer's request. Insured out of pocket expense will not exceed the insured (purchaser) policy deductible for the
work agreed upon between the insurance carrier and the contractor. Insured also agrees to provide all the necessary documentation for the claim (including documentation to facili-
tate payment from insurance carrier and/or mortgage holder). The contractor reserves the right to file for supplemental claims due to material and/or labor increases due to a storm
environment or to insurances adjuster's mistakes, errors, or omissions.

I/We agree that by signing above we agree to all of the conditions on both the front and back of this agreement.
1. Watershed Roofing and Restoration shall hold in trust any payment from the property owner until the roofing contractor has
delivered materials or has performed a majority of the work. Watershed Roofing and Restoration has a 72 hour right of rescis-
sion. Watershed Roofing and Restoration will not conduct any work on your claim within the first 3 days of this contract. After the
72 hours have passed, this proposal will become a binding contract. This contract may also be rescinded within 72 hours after
the property owner receives written notice that its insurance claim has been denied. Existing Colorado statute related to property
insurance fraud (CRS 18.13.119.5) states that paying, waiving or rebating a property owner's insurance deductible is a Class 2
Misdemeanor.
2. If material has to be reordered or restocked because of a cancellation or change by the customer, there will be a restocking
fee equal to twenty percent (20%) of the contract price.
3. A copy of General Liability and Worker's Compensation insurance has been supplied to you in our property owner packet. If
you ever need a copy, please contact the office and we will be happy to provide you additional copies.
4. Watershed Roofing and Restoration agrees to perform the specified repairs/replacement in accordance with generally ac-
cepted roofing and construction guidelines, adhering to local codes in effect at the time.
5. Payment terms: 50% payment upon delivery of material and 50% upon completion. Payment is due within ten days of receipt
of the invoice. Completion is not contingent upon inspection by the applicable governmental entity where the work is performed.
Warranty on labor begins upon completion of the project and upon full payment of the contract. Interest charges of one percent
{1%) per month will be charged on all past due balances and customer shall also pay all costs of collection including reason-
able attorney fees and costs. Watershed Roofing and Restoration reserves the right to record a mechanics lien on the property
if payment is not made in a timely manner. Unless otherwise required by law, the parties agree that the Courts for the County of
Jefferson, State of Colorado shall have exclusive jurisdiction over any legal actions. The parties agree and consent to the juris-
diction of that court over the subject matter of those controversies and over them personally.
6. Price of proposal is valid only for 30 days from proposal date based on quarterly material price increases.
7. The homeowner is responsible for getting Homeowners Association approval of roofing products and color, if necessary
8. The property owner is responsible for facilitating mortgage process and cannot hold Watershed Roofing and Restoration li-
able in delays caused by the mortgage endorsement process.
9. Property owner acknowledges Watershed Roofing and Restoration as the general contractor and as such, Watershed Roof-
ing and Restoration will be entitled to 10% overhead and 10% profit.
10. Extra material is ordered on every job to avoid issues or complications, therefore all excess materials remain the property of
Watershed Roofing and Restoration.
11. The company shall not be liable for failure of performance due to labor controversies, strike, fire, hail, weather, inability to
obtain material from usual sources, or any other circumstances beyond the control of the company.
12. Warranty: Five (5) year workmanship warranty on roof replacement, excluding 90# mineral surface rolled roofing, and one
(1) year workmanship warranty for all other work. The company will provide a service call for roof inspection as long as the
customer is the current owner of the property and all obligations are paid in full. No warranty exists until this contract is paid in
full. Watershed Roofing and Restoration is not responsible for any leaks or water damage below the roof line, including but not
limited to leaks caused by excessive wind, ice or hail during the period of the warranty. The warranty does not cover events or
circumstances not the fault of Watershed Roofing and Restoration including, but not limited to, ordinary wear and tear, improper
maintenance and damage by others. The warranty for the materials is limited to the manufacturer's warranty and the Homeown-
er agrees to look to the manufacturer for any defects or deficiencies in the materials.
13. Client agrees to not hold Watershed Roofing and Restoration liable for the following damages that may occur during roofing
construction: slight scratching to the gutters, damage to plants and trees due to roofing and tear off, cracks in driveways, interior
damage, such as split crown molding, cracked plaster or sheet rock, nail pops, or falling dust or debris, or items knocked off the
wall or shelf due to the construction. Watershed Roofing and Restoration is not responsible for pre-existing damages to any part
of the property, which includes, but is not limited to solar panels, evaporative coolers or existing skylights.
14. The price on the contract does not include replacement of deteriorated decking, fascia boards, swamp coolers, ventila-
tors, and chimneys, counter flashing or any other work completed for the property owner not stated in this contract, and will be
charged as an extra on a time and material basis.
