Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

TPRMS AND CONDITIONS
1. PAYME14T. You are personally obligated to pay the final price for all work and
materials provided by Contractor. The final price will be determined by calculating the
Replacement Cost Value (RCV) of the work performed. You will pay the RCV amount
calculated by Contractor, or your insurance company, whirhoveris greater. In addition to
this, you also agree to pay the amount of any insurance supplements and any costs,
including profit and overhead, related to necessary work not included in the insurance
company estimate but undertaken by Contractor at its sole discretion, including but not
limited to destructive testing repair and replacement ofdama property, improvements
to thepropertl; supplementsmquestedbyyou orrecommended by Contractor, orupgrades
required by code, permit or odra law. This will include profit and overhead because
Cantractorisactingasageneralcontractor. You represent toCornractorthatthepricewill
be paid byyourins raucecompany, eveWforyourdeductible, butyon agree tlatyou will
be personally liable for all anoints due to Contractor under the teats of this Agreement.
This totalmaynotbeknownwith certaintyuntiltheproject iscompicted, and so Contractor
will infonnyouofitsbest estimateofthefinal cost farpurpasesofcomputingamountsdue.
UNDER COLORADO LAW, CONTRACTOR CANNOT PAY, WAIVE, REBATE, OR
PROMISE TO PAY, WANE OR REBATE YOUR INSURANCE DEDUCTIBLE.
2. DUE DATE. Payment equal to the Actual Cost Value (Atm ofyour insurance claim
is due upon delivery of materials. If you do not receive a check for your ACV, then

4. INSURANCE.
4(a). Communication. You authorize Contractor to contact and negotiate with your
insurance company concerning facts of your claim, to £ilea a claim on your bohalt; and
determine the scope of work, Replacement Cost Value, Supplements, and all other issues
related to the work This does not include negotiation over ituwancepolicy coverage and
Contractor is not acting as your Public Adjuster. You ioshurtyoorinstsancecompanyto
cooperate with Contractor, and agree that you and your insurershall cooperate fully and in
good faith with Contractor.

address an emergency situation at your property, it may not be passible to ciatemune the

4(4 Adequacy. Contractor's obligation to perform the scope of work under this
Agreement is contingent upon your insurance company autirorizhtg sufficient funds to
complete the project If, in Contractor's sole discretion, Contractor determines that your
insurance company cannot be made to pay funds commercially reasonable and sufficient
to complete the project, Contractor may cancel this Agreement without penalty to
Contractor.

communications with your insurance company.
5. IMURIES. You agree not to enter or permit others, including pets, to enter under the
work area while wort is ongoing or to approach Contractor's equipment or materials.

WORK AREA YOU SHALL INDEMNIFY, DEFEND, AND HOLD HARMLESS

CONTRACTOR INJURED AT THE PREMISES.
6. HAZARD TESTING. Bysigtdng NsAgranment, you arerepresenting to Contractor
that, to thebestefyourknowledge, understanding and belief, yourhome does rut contain
asbestos oranyotherbazardousmaterial. Nevertheless,ifContmetordeter=nesthat a test
for asbestos orother envimnmentalhazardis necessary, you agree topaythe costs forsuch
a test and approve delay related to such testing.
7. INCIDENTAL AND CONSEQUENTIAL DAMAGES. You agree that Contractor
is not liable for any damages of any kind, whether direct, consequential, incidental, or
otherwisq resulting from the actions of Contractor or its subccnhackas, and you agree to
indetatifyandholdhamless Contractorand anyof its employees, agents, or subcontractors
fimn any claim. This includes damages related to Homeowner's Association Hiles or
penalties resultinghurnContractor'swork Contmctor'sfabilityislbnitedtoitswannnty.
S. PRE-S10STINGORHIDDENDEFICIENCiES. Contmctceisnotresponstbh for

other manifestations of hidden defects or unavoidable events associated with roofing
wok. You ackaowledge that dLq*vrzyofanysuch deficiencies orhazards may alter the
scope of the work, cause damage, increase the price of the wort, or cause delay of
additional testing, at Contractor's sole option.
9.-HANGINGITEMSANDATTACHJYIENTS. Roofing wodccreatesvibrations that
may damage lunging items, so you agree to remove items hanging it= inside and
outside wells, soffits, and ceilings. Ifit is necessary orpntdent to remove roof -mounted
equipment to perform work, you will remove and reinstall the acme at your cost.
Contractor mayremove, the equipment i f you ban not done so in order to complete work
and, if it does, will have no obligation to reinstall or align the equipment
10. WARRANTY AND LD OTATiONS. Contractor warrants that its work will be
performed in a good andworkmanlikemamer and that your wofwillpass any inspection
required by taw. This warranty lasts 5 years from the date work is complete. You agree

immediately take reasonable action toprevent any further damage and allow Contractor

maybe covetedbymanutheturer's warranties, and ifso, those warranties are unaffected
by this Agreement. Contractorwill transfer all warranties to you upon payment in full.
Contractor is not responsible for work done by others. If you have another contractor
finish or redo Contmaor's work without its written approval, including work on or
removal of solar panels, such work will void Contractor's warranty. EXCEPT AS
SPECIFICALLY WRITTEN HERE, CONTRACTOR MAKES NO WARRANTIES,
EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF"FrrNPSS OR
INERCHANTABILkTY. ALL WARRANTIES ARE VOID IF YOU BREACH THIS
AGREEMENT ORIF YOU FAD. TO MAKE FULL PAYMENT AS REQUIRED AND

progress, completed work, or the property or your personal. property. You agree to
maintain in force a policy ofhomcownces insurance covering casualty to your property
from such conditions.
12. CANCEidATTON/RESCISSION. You may cancel and rescind this Agreement
and receive a refund of any deposit paid, by delivering written notice to Contractor at
4955 Miller Street, Suite 200, Wheat Ridge, Colorado 80033, received on or before 72
hours after you sign it. You may give notice bypersonal delivery, e-mail, ornail. You
may also cancel or rescind this Agreement under C.R.S. § 6-22-104. Ifyou cancel the
Agteaneatbefore Contractor begins workon the property or orders materials you agree
to pay to Contractor the sum of 20% of the final contract price as compensation for
Contractor'seffmtsprepaingtobeginwaakand asliquidawddarrages,aminotepenalty.
If you cancel this Agreement after work: has begun, you will still pay to Contractor the
price, typically the RCV plus profit and overhead, for any work completed befre the
cancellation and such work as is necessary, in Contractor's discretion, alter the
conciliation to prevent da vW or loss to any property afterwards. If Contractor must
reorder or restock material due to cancellation other than within the first 72 hours after
the Contract is signed, then you will pay Contractor a restocldngfee equal to 15% ofthe
contract price.
14. EXCESS MATERIAL As an unavoidable consequence of Contractor's efforts to
ensure your project is properly completed, there maybe excess materials purchased If
so, those exacts materials remain the property ofCoatraetor.
15. AUTHORITY. Contractor is not obligated under this Agreement nntilitissigned
andapprovedbyaneffimrofthe company, anwAorizedagent, orworkhw commmnced

only in Arapahoe County, Colorado, or in the County in which the property is situated,
at Contractor's sole option. Upon Contractor's sole election, all such disputes shall be
resolved by binding arbitration under the rides ofthaAmeriwnArbitration Association,
with you and Contractor each paying lu fofthe costs ofan y arbitration, and Contactor
nay recover its costs to conduct arbitration fromyouin additionto any other amounts if
it prevails.
18. MODIFICATION. MgsAgmementmayonlybemodifLedlnwritingsignedbyyon
and by Contractor. The headers in this Agreement aro for organizational purposes only
and are not themselves terms of theAgreement No failure by Contractor to enforce any
provision of this Agreement will be construed as a waiver.
19. INCORPORATION. This Agreement constitutes the full and complete agreement
between the parties. You agreed that you will not rely upon and have not received any
representations by a sales representative, Contractor, or its agents except as specifically
written in this Agreement, and that no additional promises orrepresentations have been
offered as an inducement to signing.
20. I IVALIDITY. Many provision ofthisAgecmeatisimyalidunder applicable law,
it is to that extant deemed omitted. The remainder ofthis Agreement shall be valid and
enforceable to the maximum extent possible.
21. COUNTERPART'S. This Agreement may be executed in more than one
counterpart and by different people on separate co rnerpats; counterparts together
constitute the sane agreement.
