Map Revision Data. Certified documentation by a registered professional engineer of as-built
conditions for flood plain alterations were received and submitted to FEMA for a flood insurance
map revision.
Fill Certificate. A community official certified the elevation, compaction, slope and slope
protection for all fill placed in the flood plain consistent with NFIP regulations Part 65.5 for map
revisions.

._(1) This permitwas issued In accordance with the provisions set forth in your applicatidn and is subject to the laws of the State of
Colorado and to the Zoning Regulations and Building Codes of Wheat Ridge, Colorado or any other applicable ordinances of
the City.
(2) This permitshall. expire if (A) the work authorized Is not commenced within sixty (90) days from Issue date or (B) the building
authorized is suspended or abandoned for a period of 180 days.
(3) If this permit expires, a new permit may be acquired for a fee of one-half the amount normally required, provided no changes
have been orwili be made in the original plans and specifications and any suspension or abandonment has not exceeded one
(1) year. If changes have been or if suspension or abandonment exceeds one (1) year, fulifees shall be paid for anew permit
(4) No work of any manner shall be done that will change the natural flow. of water causing.a drainage problem.
(5) Contractor shall notify the Building Inspector twenty-four (24) hours in advance for all inspections and shall receive written
approval on inspection card before proceeding with successive phases of thejob.
(e) The issuance of a perrmit.or the approval of drawings and specifications shall not be construed to be a permit for, nor in
approval of, anyviolation of the provisions of the building codes or any other ordinance, law, rule orregulation. All plan review
is subject to field Inspections.

Subject: Fence at 41 st and Otis St.
Hey Chad,
Hope all is well with you. I just received some information on a 6 wood fence that is located at 41 st and Otis St.
The residence and fence are on the southwest corner of the intersection and the fence may be in violation or at
least a site distance issue. The caller also said that they did not see any permit posted on the fence or property
and was concerned about an issue related to safety due to site distance issues.
They also asked if someone could check to see if the fence met City requirements, so if you could check it out, it
would be greatly appreciated.
