Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.

L A retaining wall @ 4' +/- height had been poured on the South side of a proposed
driveway with 6" diameter piers at 10' o.c. and with a sloping back fill to the South as
shown page I of the existing calculations. The vertical reinforcing from the 6" diamete
pier doesn't have the ability to resist overturning of the wall by the regular backfill plu
the addition force created by the sloping back fill.
The solution is to install a cleadman wall @ 8' beyond (south) the existing wall and inst-
tieback steel rods and plates @ 8' o.c. as show on page 4 of the attached calculations.
2. A second retaining wall@ 4'+/- height had been poured on the North side of the I
proposed driveway adjacent to the neighbor's property line with 6" diameter piers @
10' o.c. The vertical reinforcing from the 6" diameter pier doesn't have the ability to
resist overturning nor does the pier have the ability to resist sliding of the wall due to
the regular soil pressure and the additional pressure caused by the vehicle surcharge
loading.
The solution is to again install a deadman wall @ 8' back from (south) of the existing
wall and install tie back rods and plates @ 8' o,c. plus install #4 rebar in the slab -on -
grade epoxied 6" into the existing wall as shown on page 4 of the attached calculationsi
in the repairs structurally sound.
