Purpose
The purpose of the inspection was to verify the presence or absence of asbestos in suspect building materials and
provide regulatory interpretation and guidance for compliance and protection of human health. Often times, an
asbestos inspection is requested in correlation to upcoming remodel, renovation, demolition or other rehabilitation
activities that will impact building materials that may contain asbestos. The reported project scope and date of
service for the inspection completed at the property is:

The inspecton was completed to comply with Colorado Department of Public Health and Environment (CDPHE)
regulations, specifically Air Pollution Control Divisions Regulation No. 8, Part B - ASBESTOS (Reg. 8) which mandates
the identification of asbestos -containing materials prior to beginning those activities to protect human health from
possible asbestos exposure. In addition to CDPHE, Occupational Safety and Health Administration (OSHA) requires
that building owners identify presumed asbestos -containing material (PACM) in their buildings and treat the PACM as
asbestos -containing materials (ACM) until the materials are proven not to contain asbestos. Due to the large
oversight of CDPHE in Colorado and their authorization by Environmental Protection Agency (EPA) to govern and
regulate asbestos in CO, EPA National Emission Standards for Hazardous Air Pollutants - 40 CFR Part 61, Subpart M
(NESHAP) generally do not directly appy, but are often incorporated to Reg. 8 by reference and therefore may be
applicable.
Procedure
The inspection was completed in accordance to those regulations and in a manner determined by the inspector,
based on information provided by the client, to provide interpretive data and sample results for materials reported to
be impacted and may not include data and results for all materials in a property.
The property located at the site address identified in this report was inspected by a building inspector trained by an
approved EPA Asbestos Hazard Emergency Response Act (AHERA) training organization and certified by CDPHE.
During the inspection, the inspector inspected and assessed suspect building materials, pursuant to the EPA "Green
Book," Managing Asbestos in Place, Appendix G (1990), described by the client to be affected during demolition or
renovation which would need to be sampled and analyzed for the presence of asbestos prior to commencement of
work activities that would break, crack or otherwise damage the material(s) in a manner that could allow asbestos
fibers to release from its compound, thereby creating a hazard to human health.
The inspection was completed in the levels of the property, listed below. Each material inspected and sampled may
be limited to a specific room or part of each level that will be defined in the detailed summary of each material. If
additional materials require inspection that are not included in the list below, please contact Advent Environmental
Consulting, LLC (AEC) to complete a supplemental inspection.

Representative samples of suspect materials found in the area(s), listed above, were collected. A suspect material
is any material that may have been manufactured with asbestos as part of its compound. Asbestos was commonly
used in many materials because of its resilient properties, fiberous structure, worldwide abundance and relative
ease and low cost to produce. Due to its qualities and relative abundance and low cost, many materials were
manufactured with asbestos, and still are. Generally, a suspect material is any material that is not pure, solid wood`ity of
glass, metal or stone and for the purpose of understanding this report and the completed inspection, those materials
are not inspected and should be considered, non -suspect, asbestos -free materials, when in their pure forth anNyWat Ridge
of coatings, binders, or other additives designed to enhance, protect or otherwise alter the material.
Building Division
After suspect materials are identified, interpretive data such as quantity, friability classifications, damage
assessments and potential damage assessments is collected and used subsequently to the inspection to compare
to analytical results, interpret the data, determine applicable regulations and provide recommendations for asbestos
management, if found.
Samples of those suspect materials were then collected in locations determined by a sampling scheme intended to
randomize them over the material area and provide representative data related to the unique materials present in the
material compound and possible asbestos content. Those samples were then submitted to a laboratory, along with a
chain -of -custody, accredited under the National Voluntary Laboratory Accreditation Program (NVLAP) to be analyzed
for the presence of asbestos.
Those data are recorded and reported in this document, along with sample results. Below, is a summary of findings
for the suspect materials identified, inspected and sampled. Regulation interpretation and recommendations are
provided at the end of this report. Additional non -suspect materials, other than the materials listed above, are also
listed, if any:

This report denotes the type of investigation in regard to the the destructive nature applied to verify material
identification. Destructive investigations are often necessary, especially when a Demolition Notification and
Application Form (see Regulation Overview at the end of this report) is needed which certifies that an asbestos
building inspector has inspected the area to be demolish and concludes that it is free of regulated asbestos.
Destructive investigations include opening up walls, pulling up floors and otherwise looking for hidden materials to City of

property is occupied or, at the time of inspection, cannot withstand asthetic damage) an non-destructive inves Ridge

Limitations
Although diligence was used to inspect and sample all materials included in the scope of work, there may be additional
materials not covered in this report discovered at a later time which may need to be inspected. If additional, suspect
materials are discovered which are not covered in this report, please contact AEC prior to disturbing them to have them
property inspected. ANY MATERIAL NOT PROVEN TO BE FREE OF ASBESTOS SHOULD BE ASSUMED TO CONTAI N
ASBESTOS UNTIL PROVEN OTHERWISE
AEC and its consultants, partners, agents and employees shall not be liable to the client for any materials damaged,
during the rendering of services by the AEC, in order to collect samples, or complete an investigation intended to be
destructive to verify the existence of hidden materials. Additionally, AEC shall not be liable to the client for any
materials damaged, during work activities subsequent to the services rendered by AEC, which were not properly
inspected by AEC. Finally, AEC shall not be liable to the client for any special or consequential damages, including but
not limited to, lost profits, loss of use, or any other cause whatsoever. Any compensation to the client for damages
shall not exceed the total compensation received by AEC from the client.
Material quantities listed in this report are approximate quantities of the amount of material that will be included within
the scope of work based on information gathered from the client. This approximate quantity is used by the inspector to
determine the amount of samples to collect respective to the regulatory requirements outlined in the Identifying
Homogenous Areas & Sampling section of the Glossary. Contractors using this report should gather more accurate
measurements before drawing conclusions of subsequent work activities.
This report may not be amended or reproduced, except in full, without written permission from AEC. The receiving party
shall not alter, modify, decompile, disassemble, reverse engineer, translate or create derivative works from this report.
The receiving party shall use this report only for the limited purpose described above and not for any other purpose.
The asbestos inspection was conducted in a manner consistent with the degree of care and skill ordinarily exercised
by and consistent with the standards of competent consultants of the asbestos inspection profession. The results,
findings, conclusions and recommendations expressed in this report are based on conditions of the specific areas,
defined by the client, observed during the time of the limited inspection. The information contained in this report is
relevant to the date on which this survey was performed, and should not be relied upon to represent conditions at a
later date. This report has been prepared on behalf of and exclusively for use by the client for specific application to
the project as discussed. Advent Environmental Consulting does not warrant the work of regulatory agencies,
laboratories or other third -parties supplying information which may have been used in the preparation of this report No
warranty, express or implied is made.
Detailed Report
The following pages will include a detailed report of each of the above listed, suspect materials included in the
inspection. On those pages, you'll find the designated material code, description of the material and other important
information used for result interpretation and applicable regulation determinations. Such information includes the
quantity, friability, condition and potential for damage of the material, as well as, sample information such as; sample
code, image, location and analytical result. Any numerical percentenge irritated in the 'Sample ResulC is a positive
asbestos result which may include any, regulated asbestiform including: Chrysotile (most common), Amosite,
Crosidolite, Tremolite, AMhophyllits and Actinolite. Exact asbestos identification can be found in the lab report. Please
see the glossary, at the end of this report, for definitions and more information regarding applicable terms.

Regulation Overview
Any material with an asbestos content greater than 1 % that is friable or will become friable during work activities, when those
activities exceed the below listed trigger levels for a single project, is considered a regulated asbestos containing material
(RACM) by both Environmental Protection Agency (EPA) and Colorado department of Public Health and Environment (CDPHE)
and requires certification, notification, compience with regulatory abatement sequence, air quality verification and proper
disposal at an approved, asbestos landfill. Non -friable materials that can remain non -friable during disturbance require
notification and proper disposal.
Trigger Levels
These levels are the cumulative levels of ACM in a single project and NOT amounts per material or location (e.g. 10 square
feet of floor tile in 4 separate areas = 40 square feet of surface area):

Contractor and Homeowner Advisory.
I n Colorado, unless you are a homeowner conducting work inside (not outside) your own home that is your primary
residence that is not on the market, planned to be put on the market, used as an investment property or have any upcoming
demolition that would allow the indoor air quality to mix with the outdoor air, those regulations require ANY individual
disturbing RACM to be certified by CDPHE and follow strict guidlines for proper asbestos management and protection of
human health. AEC does NOT recommend that any homeowner disturb asbestos -containing materials, although CDPHE does
allow this narrow exception. ANY HOME THAT HAS WORK DONE TO RACM BY A HOMEOWNER AND NOT A CERTIFI ED
CONTRACTOR IS CONSIDERED CONTAMINATED AND POSES AN ASBESTOS HAZARD BY THE COLORADO DEPARTMENT
OF HEALTH, FOR WHICH THE OWNER MAY ELECT TO PERFORM, BUT HOLDS THE LIABILITY OF DISCLOSURE TO THOSE
ENTERING THE HOME AND CONTRACTORS PERFORMING SUBSEQUENT WORK. CONTRACTORS WHO KNOWINGLY SEND
EMPLOYEES I NTO AN ASBESTOS CONTAMI NATED HOME ARE ALSO LIABLE TO THEI R EMPLOYEES.
Occupational Health and Safety Administration (OSHA).
Although EPA and CDPHE have NO CUTOFF DATE for the necessity of asbestos identification (.e. even materials in brand-
new buildings have to be assumed to contain asbestos), OSHA regualtes ANY amount of asbestos in buildings built before
1981 and mandates the duty of employers to protect its employees from asbestos exposure, regardless of its friability. OSHA
mandates employees disturbing ANY amount of asbestos, except when that amount is less than 10 square feet for a single
project, have basic asbestos training and participate in an annual physical, pulminaryfunction exam, and respiratory
protection plan, of which records must be maintained for 30 years.
LaharaULry Analy"
Initial results are generated from a visual estimate by the analyst and could differ with a more accurate analysis known as
pant counting. Any friable material sample identified as having a trace (equal to or less than 1%) amount of asbestos, must
be considered a RACM unless verified by point -canting to be equal to, or less than 1% and, in fact, a trace material. For
materials with a set of multiple samples (.e. SM with 3, 5, or 7 samples) the asbestos content is whatever the highest result
is out of the set; therefore, if one result of a material with multiple samples is trace, but another result of the same material is
3%, then the 3% result supersedes the other results for regulatory and asbestos management purposes.
Demolition
CDPHE requires a notice prior to demolition of any facility or facility component for any demolition project commencing in the
state of Colorado. Per CO Regulation No. 8 - Asbestos:
0.38. (definitions)
"Demolition means the wrecking or taking out of any load -supporting structural member of a facility together with any related
handling operations or the intentional burning of any facility."

unsure, please verify with your contractor, architect or engineer to determine if your project will impact a load-bearing,
structural member and require a Demolition Notification and Application Form provided by CDPHE More information about

report denotes the necessity of a demolition notice, based on information provided to AEG however, AEC shall not be liable for
changes in the scope or unknown circumstances which may, in fact, require the project to include a demolition notice. It is
the sole responsibility of the owner to determine if a demolition notice is required.
Recommendations
Analytical Result of None -Detected
Materials with a result of None -Detected have been verified by the laboratory to be f ree of asbestos fibers. Although those
materials are free of asbestos (a known and regulated hazardous air pollutant) other airborne contaminants may still cause
adverse health symptoms, both acute and chronic, if one inhales elevated amounts of particles. vigilance and caution should
be used to minimize airborne particulates regardless of the material and if visible or perceived emissions are present one
should take action to protect their respiratory health by use of personal protective equipment, or otherwise. Please work
safely and if new materials are discovered, which are not covered in this report, contact Advent to have them sampled for
asbestos.
Asbestos -Containing Material
Materials with an asbestos content greater than 1 % have been identified. Please do not disturb asbestos -containing
materials in any manner that would damage, break, crack or otherwise risk the possibility of asbestos fibers releasing from
the material, contaminating the air, and creating hazardous exposure to asbestos fibers. Asbestos -containing materials in
non -school buildings do not have to be removed simply because they contain asbestos, or managed in a way that would
prevent accidental disturbance; however, if a friable asbestos -containing material will be disturbed, or a non -friable asbestos -
containing material will be disturbed in a manner that will render it friable, or otherwise damage, break, crack or risk the
possibility of asbestos fibers releasing from the material, contaminating the air and creating hazardous exposure, it must be
managed by a certified asbestos abatement contractor approved by the Air Pollution Control Division to manage asbestos.
Trace Composited Drywall System
The asbestos content of the drywall joint compound has been averaged, in accordance with regulation, and the material has
been verified a trace material with an asbestos content equal to or less than 1 %. Joint compound, represented with code JC,
is typically part of a set including a wall texture (code SM) with the same numeric code (example: SM1 & JC1, SM2 & JC2,
SM3 & JC3 and so on, are part of the same set). If the SM samples are verified to be free of asbestos, but the corresponding
JC sample has asbestos, then the JC can be averaged over the entire drywall system which usually lowers the asbestos
content percentage to below or equal to 1 %, making it a trace material. EPA and CDPHE Regulations do not apply to trace
materials; however, there are applicable OSHA regulations which employers must abide by to protect the health of employees
performing asbestos removal.
Certification
Colorado Department of Public Health and Environment has strict requirements for the type of data and how data and
samples are collected during asbestos building inspections and does not recognize any data collected or provided for
purposes of asbestos identification and management by anyone other than a certified asbestos building inspector certified by
an AHERA accredited training provider. Anyone collecting data for asbestos identification and management who is not a
certified individual may be in violation of Colorado Regulation No. 8, Part B - Asbestos and should not be collected by anyone
other than a certified asbestos building inspector.

GLOSSARY
Material Friability
These categories were used to determine methods for assessing suspect materials based on their friability
and ability to release harmful asbestos fibers— i.e. friable materials are more likely to release asbestos fibers.
1. Friable (F) - materials that can be crumbled or reduced to powder by hand pressure.
2. Category I Non -Friable (NFI) - materials that cannot be crumbled or reduced to powder by hand
pressure including resilient floor coverings, gaskets, packing materials and asphalt impregnated roofing
materials.
3. Category II Non -Friable (NF) - All other materials that cannot be crumbled or reduced to powder by
hand pressure that do not include category I non -friable materials.
Material Type
AH ERA and Reg 8 require building inspectors to categorize the type of suspect building materials into one of
three categories:
1. Surfacing Material (SM ) - a material that is sprayed on, troweled on, or otherwise applied to
surfaces, such as acoustical plaster in ceilings and fireproofing materials on structural members, or
other materials on surfaces for acoustical, fireproofing, or other purposes.
2. Thermal System Insulation (TSI) - a material applied to pipes, fittings, boilers, breeching, tanks,
ducts, or other interior structural components to prevent heat loss or gain, or water condensation, or for
other purposes.
3. Miscellaneous (MISC) - an interior building material on structural components, structural members
or fixtures, such as floor and ceiling tiles, and does not include surfacing material or thermal system
insulation.
Friability and material type categories for each material inspected are reported with their respective friability
and type codes (e.g. F -SM, F -TSI, NFI, etc.). Non -friable category II materials are identified with non -friable
code (NF) and the type code for the material (e.g. NF -SM, NF -TSI, NF-MISC).
Identifying Homogenous Areas & Sampling
Homogenous areas of each material are identified within the work area. Homogenous materials are grouped
based on color, texture, and date of application. These materials are then given an approximate quantification
of removal or disturbance during work activities to determine the amount of samples to take for each material
as defined by AHERA and Reg. 8:
1. Surfacing materials
• Minimum of 3 random samples for quantities under 1,000 ft'
• Minimum of 5 random samples for quantities > 1,000 ft' & <5,000 ft'
• Minimum of 7 random samples for quantities >5,000 ft'

inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
' inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed .by City of

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In oder to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in

responsibility and will become part of this agreement.

HOA DISCLOSURE: Please note that it is the sole responsibility of the homeowner to coordinate with the HOA to abide by all HOA rules and
