Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.

2. This permi all expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.

This application is exclusively for new permits for residential roofs and for licensed

contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Contractor will hold in trust any payment from the Owner(s) until we have delivered materials
to the jobsite or have performed a majority of the roofing work on the property.
2.2 The owner(s) and contractors acknowledge that the Owner(s) is responsible for his/her deductible
and any cost associated with changes made to the Scope over and above the approved Estimate.
Owner(s) is responsible for any and all approved cost and fees not paid by the provider. First
General Services of Denver cannot pay, waive, rebate or promise to pay, waive or rebate all or part
of any insurance deductible applicable to the insurance claim for payment for roofing work on your
property. At no additional cost to the Owner(s), First General Services of Denver also reserves the
right to file supplemental insurance claims due to material and labor price increases due to storm
environment. 2.3 Any and all changes to this Contract will be addressed in a Change Order to be
signed by both the Owner(s) and Contractor. All parties acknowledge the Change Order as part of
this Contract
Miscellaneous:
1. Each party hereby agrees that he/she has read this agreement carefully and fully understands
that the terms of the same.
2. Each party agrees that the other has given sufficient consideration to the other for the benefits
and restraints provided for in this contract.
3. Neither party may assign its rights or delegate its duties under the Agreement, unless such party
shall first obtain written consent of the nonassigning party (which consent may be withheld in the
nonassisning party's sole and absolute discretion).
4. This Agreement shall be binding upon and inure to the benefit of the parties to this agreement
and their respective successors and assigns.
S. The article and section headings are inserted only for convenient reference and do not define,
limit or prescribe the scope of this Agreement or any exhibit attached hereto. The Recitals of this
Agreement are included as an aid to interpretation of this Agreement, but do not themselves create,
limit or define any rights or obligations of the parties hereto.
6. Should it become necessary for First General Services of Denver to employ an attorney to enforce
any of the conditions or covenants hereof, including the collection of any unpaid amounts due to
First General Services of Denver or upon material breach by Owner(s), Owner(s) agrees to pay all
expenses so incurred, including any and all costs and reasonable attorneys' fees if judgment is found
in favor of First General Services of Denver.
7. This Agreement shall be governed by and interpreted in accordance with the laws of the State of
Colorado. The parties hereto consent to venue and jurisdiction in the District Court in and for
Douglas County, Colorado, or in the United States District Court for the District of Colorado in any
action commenced relating to this Agreement or the transactions contemplated hereby.

All material guaranteed to be specified and above work to be performed in accordance with specifications submitted for above work
and completed in a substantial workmanlike manner. Warranties may be transferrable upon request at the discretion of First General
Services of Denver. Labor warranties are void if roof has been tampered with by anyone other than First General Services of
Denver. All warranties void if payment is not made when due. This agreement is contingent upon strikes, accidents, weather or
delays that are beyond First General Services of Denver control. Owner(s) to ca; ry fire, tornado and other necessary insurance
upon above work. Workers compensation and general liability insurance shall be carried by First General Services of Denver. The
contract is the entire agreement between the Owner(s) and Contractor and can only be modified with a separate written agreement
signed by both parties.
Owner(s) has the right to rescind this contract and obtain a full refund of any deposit within 72 hours after entering into this contract.
If you plan to use the proceeds of a property and casualty insurance policy to pay for the roofing work you may rescind this contract
within 72 hours after you receive written notice from the property and casualty carrier/insurer that your claim has been denied in
whole or part. However, First General Services of Denver is entitled to retain payments for deposits to compensate First General
Services of Denver for roofing work actually performed in a workmanlike manner consistent with standard roofing industry practices.
Owner(s) may cancel this contract by providing the Contractor a signed, written notice at any time prior to 72 hours after the time of
this transaction by delivering in person or US Postal Service mail to PO Box 470727, Aurora, CO, 80047. After 72 hours, there will
be a 15% cancellation fee.
First General Services of Denver is not responsible for any existing cracked or broken driveways. Verbal understanding and
agreements with representative shall not be binding. All understanding and agreements must be set forth in writing on this contract.
Owner(s) agrees to remove breakables from outside wall of home during installation of work. First General Services of Denver is not
liable for interior damage due to normal construction vibration and extreme weight load changes and is not responsible for any
damages below the roof due to leaks by wind, ice, hail or poor building structure during the period of the warranty. Owner(s) is
responsible for complying with their homeowners association and architectural control requirements. Contractor will not be
responsible for unapproved materials installed. Skylights are not warranted unless First General Services of Denver replaces
them -All materials are the property of First General Services of Denver.
For purpose of this agreement, in the event that the work to be provided hereunder requires a final and approval by a local building
authority,the work to be performed hereunder by the Contractor shall be considered completed when local building authority that the
work has passed final inspection. The Contractor shall correct any deficiencies following such inspection that is required by the local
building authority in order for the work to pass final inspection. The Contractor is not responsible for any delay in final inspection by
any local building authority after the Contractor notifies the applicable local building authority the work is finished and ready for final
inspection.
