Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

contractors only. This type of permit is ONLY being processed online --do not come_
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE

herein as ARC.will perform the repairs or repiacernent specified by your
insurance company for insurance funds only, without. any additional cost to
you, except for your deductive and any upgrades or additional work
authorized by you that are not part of your insurance claim, ARC cannot pay,
waive or rebate deductibles. Supplemental claims billed by ARC and

ANY REPRESENTATION, STATEMENT, OR OTHER COMMUNICATIONS NOT WRITTEN IN THIS AGREEMENT, OR MADEIIN WRITING PURSUANT
THERETO, ARE AGREED TO BE INVALID, AND NOT RELIED ON BY EITHER PARTY AND, DO NOT SURVIVE THE EXECUTION OF THIS AGREEMENT.
1. This agreement is subject to the credit and pricing approval of the management of AMERICAN ROOFING & CONSTiRUCTION, LLC.
2. Payment is due for Materials/Permits at Contract. Payment is due for the remainder of the agreement with completion of each individual trade, including
applicable overhead & profit, such as: Roofing, Siding & Trim, Gutters, Awnings & Carports, Interior Repairs, Punch list. In the event a final inspection is
required before the "Customer" chooses to pay. The "Customer" shall agree to pay the total balance due, less 5% as hold -back until the final inspection and
any subsequent punch list is completed.
3. Should default be made in payment of this agreement, a lien will be placed on the property and charges will be added from the date of substantial
completion at the maximum allowed by law. If placed in the hands of an attorney for collection, the "Customer" will pay all attorney and legal fees.
Colorado: Any person or company supplying labor or materials for this improvement to your property may file a lien against your property if that person or
company is not paid for their contributions.
4. The "Company" and the "Customer" agree to settle all disputes in accordance with the laws of Colorado and the county in which the agreement is executed
5. American Roofing is due the full amount of all money as specified by the Insurance Contract or by the Full Insurance Scope Amount, that is necessary to
complete the repair process as required by local building codes, and building officials, or to satisfy cosmetically, the "Customer". A change order will be
required for any work outside of the Contract and/or the Insurance Scope.
6. If the "Customer" chooses not to pay for a part of the scope of the agreement, an individual line item, or a trade, the "Customer" released the "Company" of
its obligations for the perfbrmance of that component with regard to the integrity of the building system as a whole.
7. The "Company" is not responsible for damage below the roof due to leaks caused by excessive wind greater than 55mph, ice damage, or hail at any time
during the construction process, or the warranty period.
8. The "Company" will not be responsible for slight scratching and denting of gutters, oil droplets on driveways, hairline fractures in concrete, flower, or
minor broken branches on plants or shrubbery.
9. This agreement cannot be canceled once negotiations begin with your insurance company, and/or, material has been delivered to your job -site without
written mutual agreement of both parties.
10. If this agreement is canceled by the customer, "Customer" agrees to pay to the "Company" 25% of the agreement price for: 1) Consulting Services
Rendered, or 2) Liquidating fees. If materials have to be returned due to this cancellation, the "Customer" agrees to pay an additional fee of 25% for re-
stocking fees.
11. The "Company" reserves the right to supplement the Insurance Company for increases in the "Full Scope of Work", and, or, documented price increases.
"Customer" agrees to allow these supplements to be paid directly to "Company"
12. All excess materials remain the property of the "Company"
13. "Full Scope of Insurance Proceeds" shall be defined as the full price for repairs and replacements allowed by the insurance company.
14. If any provisions of this a;geement should be held invalid or unenforceable, the validity and enforceability of the remaining provisions shall not be affected
thereby.
WARRANTY
OUR STANDARD WORKMANSHIP WARRANTY IS FOR FIVE (5) YEARS. IN ADDITION, WE WILL PROVIDE YOU A WRITTEN WARRANTY
UPON RECEIPT AND VERIFICATION OF YOUR PAYMENT IN FULL FOR THIS AGREEMENT. COPIES ARE AVAILABLE FOR YOUR PERSUAL.
15. We will not honor our warranty on agreements that are not paid in full.
