Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your

Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

WE HEREBY SUBMIT SCOPE OF WORK FOR
MAIN HOUSE:

1. Removal of all existing roofing and flashing membranes to the plywood deck surface.
2. Replacement of any damaged or deteriorated plywood decking at an additional cost of $40 per four foot by eight foot
sheet of plywood needed. Decking will be replaced in whole sheets only in accordance with recommendations by both
the National Roofing Contractors Association (NRCA) and the American Plywood Association (APA). New decking shall be
APA rated for structural use. Deck fastening will meet or exceed local building code requirements.
3. Replacement of following flashing materials: perimeter drip edge, step flashing and pipe jacks.
4. Ice and water protection membrane shall be installed along all eaves and valleys and shall extend past the interior wall
line a minimum of 24 inches to provide protection against damage from ice dams.
5. Installation of one layer of #30 asphalt saturated roofing underlayment on deck surface not covered with ice and water
protection material. Felt will be fastened using staples or 1 % inch plastic -capped. Fasteners shall be at maximum of 12
inch centers along the side and end laps and at minimum 24 inch centers in two staggered rows in the center of the roll.
6. Installation of new algae -resistant shingles. Shingles will be installed in strict accordance with the manufacturer's
specifications and shall be fastened using six nails per shingle.
7. Paint all vent flashings, roof penetrations, and chimney flashings.
8. Ventilation of attic will be installed to code using ridge vent or mushroom vents.
9. Installation of manufacturer's pre -made hip and ridge material. Hip and ridge rows will be run straight and shall be
installed according to manufacturer's instructions.
10. Remove and replace 5" gutters and 2 x 3 downspouts.

Contract Terms and Conditions
Rise and Fall / Disclosure: The quotation is based on costs and statutory obligations current at the time of the quotation and is liable to
variation. Any variation in price either prior to or after conclusion of the contract may be added to or subtracted from the estimated price.
Grounds ! Electrical Power: All grounds to be cleaned up on a daily basis. All bushes, shrubs, and flowers to be protected. Homeowner is
asked to supply electrical power if needed.
Insurance: Frailey Roofing is in compliance with all state and county requirements for general liability and workers compensation insurance.
Payment: Sixty percent (60%) of Total Estimated Cost is due prior to start. Remainder 40% of total estimated cost is due immediately upon
completion of project. Final payments exceeding two weeks will be subject to 3% late fee. Frailey Roofing shall hold in trust any payment from
the property owner until Frailey Roofing has delivered roofing materials to the jobsite or has performed a majority of the roofing work on the
property. If project is being completed due to home closing, Frailey Roofing can accept payment from Title company at time of closing.
Permit and Final Inspection: Frailey Roofing will pull re -roof permit. Frailey Roofing is responsible for calling in final inspection.
Dates of Service: Frailey Roofing will notify contract holder of approximate dates of service, which are subject to change.
Failure to Pay: Failure to pay in full by contract holder shall entitle Frailey Roofing to recover the value of all goods supplied and work
performed to the date of release (including lien). Failure to pay also entitles Frailey Roofing reimbursement from property owners, for legal fees
associated with cost of recovery of goods supplied and work performed.
Insurance Claim Deductibles: If the property owner plans to pay for the roofing services through an insurance claim, Frailey Roofing cannot
pay, waive or rebate the homeowner's insurance deductible in part or in whole per SB38 - The Consumer Protection/Roofing Bill, effective
06/06/12.
Warranty: Labor performed hereunder will be free from defects on workmanship, in respect to leakage, for period of 5 years from date of
purchase. Warranty is non-transferrable. Warranty is not valid unless paid in full.
Cancellation: Cancellation of any order must be notified by the contract holder to Frailey Roofing, in writing, within 72 hours days of contract,
and prior to the first delivery. If an order is cancelled the contract holder / property owner undertakes to reimburse Frailey Roofing for any
expenses reasonably incurred in the preparation or the execution of the order. If property owner rescinds the contract for roofing services,
Frailey Roofing will issue a full refund of any deposit within 10 days after rescission of the contract.
This services contra is accepted under the specified cost, terms and conditions noted within.
