Approved per plans and red -line notes on plans. Must comply with 2012 IRC, 2017 NEC and all
applicable City of Wheat Ridge Municipal Codes. Work is subject to field inspections.
As part of the foundation inspections and PRIOR TO proceeding with any further
construction/inspections, a completed, signed and sealed, Foundation Setback and Elevation
Certification shall be submitted to the City for review. The certification form shall be
fully completed by a Professional Land Surveyor licensed in the state of Colorado, stating
that the foundation was constructed in compliance with all applicable minimum setback and
elevation requirements of the City.
Prior to project completion or granting of a Certificate of Occupancy, an As -Built Survey
or Improvement Survey Plat shall be submitted to the City.
Owner/Contractor is responsible for locating property lines and constructing improvements
according to the approved plan and required development standards. The City is not
responsible for inaccurate information submitted within the plan set and any construction
errors resulting from inaccurate information.
For one- or two-family dwellings, the first twenty-five feet (25') of driveway area
from the existing edge of pavement into the site shall be surfaced with concrete, asphalt,
brick pavers or similar materials. A right-of-way permit may be required; contact Public
Works Department at 303-235-2861.

This letter is written to detail the recent steel helical pile installation at the referenced site. A total of 34
Heli-Pilem helical piles were installed to support foundation elements of the house.
The piles were installed according to the manufacturer's recommendations and engineer's specifications
by certified installers to depths ranging from 10 to 32 feet with 5,000 to 11,000 Ft -Lb of torque each.
Standard embed bracket attachments were installed to transfer the load into the helical pile. The helical
piles have minimum ultimate load capacities ranging from 50 kips to 110 kips each.

Approved per plans and red -line notes on plans. Must comply with 2012 IRC, 2017 NEC and all
applicable City of Wheat Ridge Municipal Codes. Work is subject to field inspections.
As part of the foundation inspections and PRIOR TO proceeding with any further
construction/inspections, a completed, signed and sealed, Foundation Setback and Elevation
Certification shall be submitted to the City for review. The certification form shall be
fully completed by a Professional Land Surveyor licensed in the state of Colorado, stating
that the foundation was constructed in compliance with all applicable minimum setback and
elevation requirements of the City.
Prior to project completion or granting of a Certificate of Occupancy, an As -Built Survey
or Improvement Survey Plat shall be submitted to the City.
Owner/Contractor is responsible for locating property lines and constructing improvements
according to the approved plan and required development standards. The City is not
responsible for inaccurate information submitted within the plan set and any construction
errors resulting from inaccurate information.
For one- or two-family dwellings, the first twenty-five feet (25') of driveway area
from the existing edge of pavement into the site shall be surfaced with concrete, asphalt,
brick pavers or similar materials. A right-of-way permit may be required; contact Public
Works Department at 303-235-2861.
Prior to issuance of a CO, a total of 4 trees shall be planted within the front and side
setbacks facing the street.
In addition, no less than twenty-five percent (25%) of the gross lot area and no less than
one hundred percent (1000) of the front yard shall be landscaped.
All landscaping shall be in conformance with Section 26-502 of the Zoning and Development
Code. All landscaping shall be installed in accordance with the approved landscape plan
prior to C.O., a landscape escrow agreement with financial guarantee in the amount of 125%
of the cost of the installation shall be required.

On behalf of the Northwest Lakewood Sanitation District (NWLSD), Martin/Martin Inc., acting as the District's
Consulting Engineer, offers the following in response to the site plan received December 6, 2019, concerning a
proposed single-family home at the above referenced address. The Northwest Lakewood Sanitation District has no
objections with the proposed development conditional on the items identified herein are fully addressed. The
following are general requirements for sanitary sewer service. Rules, Regulations and Standards of the District must be
complied with at all times.
The subject lot appears to be entirely within the service and boundary area of the Northwest Lakewood Sanitation
District. Treatment of sewage generated within the Northwest Lakewood Sanitation District is provided by Metro
Wastewater Reclamation District.
Note, sanitary service to mainline connection must be with a Wye
Existing District Sanitary Sewer Mainline(s)
Northwest Lakewood Sanitation District has the following sanitary sewer mains in the vicinity of the proposed
property:
• 8 -inch mainline running north to south, east of the referenced property within Union Street.
• 8 -inch mainline running west to east, north of the referenced property within W. 35`h Avenue.
It appears the property may be serviced by gravity flow; however, the owner/developer is responsible for determining
horizontal locations and vertical depths of the existing sanitary sewer main(s) and potential conflicting utilities, to verify
if service and gravity flow is feasible or if a private individual sewage ejector (lift station) is required. The issuance of a
sewer tap permit by the District does not in any way guarantee that the property can be served by gravity flow. No
underdrain or storm sump pump connections to District facilities are allowed.
Utility Locates
It is the developer's responsibility to provide the District, c/o Martin/Martin Inc., with an ASCE Utility Quality B utility
locate mapping stamped by a Colorado PE as required in Colorado Revised Statue Title 9 Article 1.5 and ASCE 38
standards. Depending on the design, an ASCE Quality Level A supplemental survey may be required which necessitates
physical contact and survey of the existing utility at crossings. All projects must meet Colorado Revised Statue Title 9
Article 1.5 for proper notification to the Notification Association by the Developer's professional engineer. It is also the
owner's responsibility to include electronic locators on all new underground facilities, including service lines up to the
structure or building being served. Submit details for tracer wire and associated test stations.

A water provider letter verifying water meter sizing for the building will be required prior to a tap application
completion. Full address of sanitary service is also required. A tap application is available online and must be submitted
to the District for review. If credits for SFRE's are being requested for existing units(s)/buildings, the Developer must
provide existing addresses, meter size, and if applicable, abandon the sanitary tap at the mainline (District observation

Approval of this building permit is subject to the following conditions and notes:
In the course of this division's review, Engineering notes two issues of some
importance for the application.
Overall Site Drainage
The applicant has procured the services of a professional civil engineer licensed
in the State of Colorado to design and certify protective measures to provide
site grading and drainage. Engineering Division will review the design but shall
not delay issuance of the building permit. If subsequent changes and reviews
are necessary, this can occur following Building Permit issuance, but the
engineering design shall be approved, and the construction completed and
certified by the professional engineer prior to issuance of the C.O.
2. Public Improvements
The Municipal Code of the City of Wheat Ridge (Sec. 26-417) provides that
street improvements "may be required as a condition of approval for a
subdivision or development application." In the course of the initial review,
Engineering has determined the following:
EXISTING CONDITIONS
• The site is bounded by West 35th Avenue on the north and Union Street on
the east.
• There currently exists 2'-8" wide sidewalk with mountable curb and gutter
along the north.
• There currently exist two ADA ramps at the southwest corner of W. 351h
Avenue and Union Street.
• There currently exists 4'-0" wide sidewalk and mountable curb and gutter
along the west side of Union Street.
PROPOSED CONSTRUCTION
Applicant currently proposes to connect to W. 35th Avenue with a new driveway.
The new driveway shall be a Type 2 or Type 3 CDOT standard driveway (See
Page 1 of 2
Standard Plan No. M-609-1, attached). This driveway shall be constructed in
compliance with Federal ADA guidelines and shall transition from the existing
2'-8" wide sidewalk east of the northwest property corner through a 5' wide
sidewalk section across the drive cut to the east.
The substandard curb, gutter and sidewalk along W. 351h Avenue will need to be
addressed — refer to Municipal Code of Laws (Section 26-417). The options
available to the applicant are as follows:
• Construct current city standard 5' wide attached sidewalk and 6" vertical
curb and gutter along W. 351h Avenue from the east edge of the new
driveway connection eastward, terminating at the western edge of the
existing ADA ramp on W. 35th Avenue at Union Street.
OR
Do not construct new curb, gutter and sidewalk and provide Fees in Lieu of
Construction ("Fees"). Fees shall be in the amount of $4,641.21 (see
attached Fee Calculation Worksheet) based upon the prevailing costs for
such construction in the City and are due at the time of Building Permit
issuance. Note that these fees are for only the length of curb, gutter and
sidewalk from the east side of the new driveway to the western edge of the
ADA ramp at the corner of W. 35th Avenue and Union Street.
Note: Should applicant feel aggrieved by the code requirements as outlined
above, applicant may pursue relief through a hearing before the Board of
Adjustment as stated in City Code Sections 2-61, 5-45.
In the course of meeting with the applicants on January 27 and through interoffice
discussions with staff, Engineering Division has made adjustments to the previous
Conditions of Approval and are as follows:
• Corner Ramps
No new work on the existing ADA ramps at the intersection of W. 351h
Avenue and Union Street or fees associated with upgrading these will be
required at this time.
• Union Street
No new work on existing curb, gutter or sidewalk, or fees associated with
upgrading the improvements are required at this time.

GENERAL
American Geotechnical has completed a subsurface investigation of the site known as:
3491 Union Street
Wheatridge, CO
This report presents the results of that investigation. The investigation was performed at
the request of John and Peggy Basil in order to comply with local requirements and to
determine the allowable foundation construction at this site.
This investigation was not intended to check for any chemicals or hazardous materials.
This report may contain references to structural components not necessarily a part of this
construction. These references are included in the unlikely event that a structure is built
that is other than that which was specified to us.
Please read this entire report. Frequently overlooked are details regarding the
requirements for open hole inspections and the use of float in the construction of
non-bearing partition walls. Please ensure that these sections and all other sections
are fully understood. If there are any questions regarding any part of this report
ask for clarification.
SITE DESCRIPTION
The site is vacant at this time. The existing vegetation consists of domestic grasses,
weeds, trees and shrubs. The site slopes generally to the south at an approximate 5%
grade until the last i/4 of the lot, where it slopes approximately 50%.
PROPOSED CONSTRUCTION
It is our understanding that a single-family residential structure is planned for
construction on this site. This structure will be constructed over a walkout basement.
This report is written based upon with the type of construction presented by the client,
modified by any soil limitations present, and is ONLY applicable for this particular
construction. If additional soil data is needed, beyond what is shown herein, positively
contact this office before proceeding.
FOUNDATION RECOMMENDATIONS — NEW FOUNDATIONS
These recommendations are applicable for conventional construction only. Foundations
deeper than 11 feet are not considered conventional. With respect to the soil
investigation performed at this site, and the intended structure as described by the client
named above, the following is one or more of the allowable foundation type(s).

Drilled Concrete Piers
This foundation may consist of a grade beam and concrete pier foundation system.
These drilled piers must meet the following criteria:
They must be a minimum of 28 feet in length, and;
They must penetrate a minimum of 18 feet into bedrock, or to refusal.
Piers may be longer than the specified minimum due to the bedrock penetration
requirements. This is especially true for shallow foundation walls such as garage walls
These piers must be a minimum of 12 inches in diameter. The maximum allowable end
bearing pressure for these piers is 25,000 psf, and there is a corresponding 2000 psf side
shear value available for the portion of the pier within bedrock.
If water is encountered during or after drilling the piers, measure must be taken to keep
the holes dry while placing concrete, or, concrete may be placed by pumping it into the
very bottom of the holes and forcing the water out. Casing may be required to eliminate
collapsing sidewalls or to minimize any infiltration of water. Piers must be poured as
soon as possible after drilling to ensure the integrity of the drilled holes. Ensure all loose
material has been removed from the pier holes prior to placing concrete.
Piers must be vertically reinforced throughout their length using steel rebar, according to
the following schedule:
One #5/60 rebar for every 16 inches of pier circumference, or a minimum of 3 #5/60
rebars, whichever is greater. This rebar must extend into the foundation wall a
minimum of 2 feet.
There must be a minimum 6 inch void space under grade beams to separate the grade
beam from the soil.
Reminder: In cases of deep bedrock and/or shallow foundation walls, piers may
ultimately be longer than the specified `minimum' length, due to bedrock penetration
requirements.
WATER
No free water was encountered, but is often encountered at shallow depths in this area.
While excavating, look for soil which `pumps' from construction traffic, or any evidence
of soft soil conditions, or free water, or very moist soil conditions. Contact this firm if
any of the above are found.
Due to the presence of expansive clay soils, and in order to minimize the chances of
water damage to the structure(s), an exterior perimeter drain is required for any
structures built on this site. This drain may flow into a sump pit fitted with a pump, or
may `daylight' to an exterior outfall location. Where a sump/pump is utilized, the outlet
line from the pump must reach an unobstructed termination point above ground and well

away from any structure. If an exterior outfall location is selected, ensure that the drain
opening is protected by a screen.
See the Perimeter Drain detail later in this report.
Ensure that the excavation is adequately sized for the safe installation of an exterior
drain, if required.
OTHER FOUNDATION RECOMMENDATIONS
On narrow lots allow enough room for construction of foundation, footings, window
wells, exterior perimeter drain, and any shoring system.
Ensure that any new construction does not intercept the bearing plane(s) of the adjacent
structures, unless an engineered shoring plan is developed and followed. This bearing
plane extends out and down at a 45 degree angle (1 to 1 slope) from the base of a
foundation.
The case equivalent fluid pressure at this site is 58 pcf. Pressure can result from the
presence of expansive soils — soils which can exert additional pressures when wetted.
SHORING
Depending on the proximity of the excavation to the property lines, shoring of the
excavation may be required to protect adjacent property. Where space is limited, site
excavation might otherwise result in slopes steeper than 1 to 1. In which case, shoring is
required. On sites of limited size or where local regulations warrant, contact the local
building agency for requirements. If shoring IS required due to proximity, positively
contact this office for engineered shoring specifications. There is an additional charge
for the specifications.
For other locations, follow OSHA requirements for proper shoring or benching of
excavations. These soils are considered Type B soils by OSHA.
Any shoring design shall take into account the proximity of adjacent structures, and
depth of adjacent foundations. Shoring should not be constructed closer than four feet,
from the centerline of the shoring to adjacent foundations.
Never excavate into the bearing plane (bearing soils) of existing foundations.
FIELD AND LABORATORY ANALYSIS
A test hole was drilled in the vicinity of the proposed structure (see Site Map) using a
Mobile B53 drilling rig and/or manual equipment. Test holes are generally drilled to a

depth of 20 feet. Holes may have been drilled to greater or lesser depths if bedrock or
water restrictions exist, or if additional depth is needed to make bedrock determinations.
In-situ soil samples were retrieved from depths appropriate to adequately anticipate
foundation requirements. These samples were retrieved utilizing a 2 -inch California
sampler. In-situ samples consist of relatively undisturbed soil. Additional loose samples
may also have been brought to the laboratory in order to make a determination of the soil
types encountered at depths other than those where in-situ samples were retrieved, or to
indicate where changes in soil composition occur as a function of depth. These samples
were returned to the laboratory for analysis.
Laboratory analysis consists of a determination of moisture content, density, swell and
consolidation potential, and particle size distribution, where possible. In order to assure
valid results, non -cohesive, highly granular samples, may not have been tested for swell
or consolidation potential as valid results of same are often not possible, or are
inaccurate.
Subjective analysis of the soil conditions was also made in the field during the drilling
operations. Investigations performed on nearby sites may also have been considered in
these results.
The following represents the results of the field observations, the laboratory analysis as
well as our experience in the area and adjacent data.
SUBSURFACE CONDITIONS

SLAB RECOMMENDATIONS
The Slab Performance Risk at this site is judged to be `high' (defined as 5-8% swell with
a 500 psf surcharge). These natural soils are not suitable for the construction of most
slabs. It must be understood that the risk of slab movement does exist. Changes in the
water content of these soils may cause the soil to swell or shrink, which may cause

movement or cracking of slabs. Potential water sources include but are not limited to:
water line breaks; excessive precipitation; irrigation system malfunctions; over -watering;
negative site grading (toward the structure); a rise in a water table; a leaking hot water
tank, hot tub, or water feature; a nearby septic system; downspout malfunctions or
incorrect gutter/downspout construction; failure to provide a pump in a sump pit.
Any potentially finished or partitioned room areas must be constructed using a
structural floor system over a void space, rather than a slab.
(Note: So-called `structural slabs' which bear on soil, are still slabs -on -grade, and
are not to be considered a structural floor system.)
GENERAL SLAB INFORMATION
The following is general information regarding slabs -on -grade, and must not be
construed as an approval, release or exception, to construct slabs or basements. It is
simply general information which describes and can limit the damage to structures
which can result from the construction of slabs. Vertical movement of slabs and the
force they can exert on lower partition walls and thereby the structure above, is the #1
cause of foundation and structural damage in the Front Range area.
Slabs should bear on undisturbed natural soils, or on properly conditioned and re -
compacted soils. If the on-site soils are potentially expansive or may consolidate, it is
preferable to bear slabs on non -expansive imported materials - materials which will also
minimize the transfer and/or retention of any water below the slab. Materials with a
Liquid Limit below 30 and a Plasticity Index less than 15 are preferred. The use of
gravel may allow undesirable migration of water below the slabs.
Slabs shall be isolated from foundation components by slip joints constructed to allow
the independent movement of the slab. Slabs shall also be separated from any utility
components by isolation joints. Mechanical equipment resting on slabs must be fitted
with expandable / collapsible sections in order to allow movement of the slab without
damage to the equipment or to the structure.
Non-bearing partition walls constructed over floating slabs must be constructed with a
minimum of 4 inches of `float' to allow for movement of the slab without damaging any
part of the structure above. Float shall be built into the framing AND drywall AND
trim. See the Float Diagram later in this report. All doors similarly built shall be
constructed with at least a 1 inch gap at floor level. Door trim must also be isolated from
slab movement. It is the owner's responsibility to monitor and maintain all floats and
gaps as necessary. Garage door trim must be isolated from floating garage slab
movement. If additional information on float is needed, please contact this or any
geotechnical office. See Fig. 2 — Float Detail - later in this report.

Slabs must be appropriately reinforced to resist the anticipated loads as well as the
effects of the supporting soils. Floor slabs must also be scored in accordance with the
American Concrete Institute (ACI) recommendations in order to control cracking of the
slab due to shrinkage or other factors. These scores should be a maximum of 12 feet
apart.
CAUTION: Bathrooms constructed on slabs in basement levels involve unique risks.
They are difficult to install without inadvertently creating a solid connection between the
supporting slab and the first floor framing above. Tile, tubs, and showers may create a
solid `stacked' condition. As such, if the slab moves upward, this may cause cracking,
bending, and breakage of floor tile, wall tile, the tub body, plumbing connections and
fittings.
Serious consideration should be given to any bath installations in basement levels.
Discussion of the above between owner, builder and/or buyer is important.
The decision to construct basement baths is a risk taken by the owner.
If some movement of a slab is not acceptable, any potentially finished or partitioned
room areas should be constructed using a structural floor system over a void space,
rather than a slab.
SITE PREPARATION
We anticipate that additional grading may be necessary. Site preparation procedures should
be performed such that the potential for accumulation and standing of water around the site
is minimized. Clay soils should be kept from desiccating (drying and shrinking) by
sprinkling the soils occasionally. Drainage swales or channels should be created where
necessary in order to route water away from the excavation and construction area.
BACKFILL
The backfill placed around the foundation walls should be methodically compacted to help
reduce settlement after completion of construction. The top one foot of the backfill
material must be relatively impervious to aid in routing water away from the foundation.
This is accomplished by the addition of clay to the top 12 inches of soil, if needed, or
simply by using clayey soils in this layer. Installing landscaping fabric may be useful,
because landscaping fabric restricts downward movement of water (through it) and causes
water to be shed away from the structure.
The on-site soils may be wetted prior to being used as backfill - to reduce their expansive
properties, where present — and to aid in compaction. Do not add water to backfill soils
already in place. The on-site soils, exclusive of topsoil and organics, are suitable for use as
backfill. First floor framing should be constructed prior to the backfill process, or, the
foundation walls should be supported by bracing or some other means.

SURFACE DRAINAGE
The area surrounding the structure should be graded so as to provide adequate drainage
away from the structure. This is accomplished by creating a 10 percent slope (12 inches
vertically for every 10 feet horizontally) down and away from the foundation in soil
areas, and a minimum 4% slope on hard surfaces. Low spots should be filled in order to
prevent accumulation and standing of water near the structure.
Surface water running toward the structures from up slope areas should be diverted around
and away from the structures by means of drainage swales or other similar measures.
Decorative edging should be constructed to allow water to flow through, around or under
the edging and not so as to cause damming of water behind the edging.
Sprinkler systems should be constructed so as to keep the area within 5 feet of the
structure as dry as possible. Sprinkler heads or other irrigation components should not
be placed in, nor should they release water into, the area that is within 5 feet of the
structure.
Downspouts must terminate outside of the backfill area of the structure and a minimum
of 6 feet from the structure. Downspouts should discharge well beyond the limits of the
backfill. The use of long downspout extensions in-place of splash blocks is advisable.

UTILITY LINES AND PIPES BELOW STRUCTURAL FLOORS
All utility lines such as sewer, water, heating systems, etc., must be protected from the
expansive soils below them. These lines must be supported by the floor above, such that
they are at least as high above the soil as the `void` or `float' amount — whichever is
greater - specified above. Pipes and lines that are elevation specific, or rely on specific
slopes or grades, cannot be allowed to come into contact with expansive soils, which
may lift them and negatively affect their function, or even crack or break them.
MOLD
Mold has become an issue in building construction. The health risks associated with the
vast majority of mold spores are minimal or non-existent, however, the debate continues
and litigation on this subject is increasing. The use of proper drainage techniques
outside of the structure, and the use of subsurface perimeter drains, should keep water
from accumulating in a way which would trigger mold formation. However, the most
current building practices which reduce the likelihood of mold formation, should be
gathered, studied and followed.
LIMITATIONS / DISCLAIMER
In any subsurface investigation, limited data is available from which to formulate soil
descriptions and generate recommendations for foundations and related construction
components. The samples taken are indicative of the subsurface materials at the time
and at the location the samples were taken. Precipitation, seasonal changes, and
excavating are just a few of the factors which may create changes in the composition of
the site. If conditions are encountered which are significantly different from those
described in this report, contact this office before proceeding.
This report does not address nor was intended to address any environmental issues,
hazardous materials or toxic waste issues, or other subsurface situations or conditions
other than those described within this report.
This report is intended for the sole use of the above named client and their approved
