Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped
off to the Permit Desk at the City of Wheat Ridge. The report MUST BE SIGNED by the
Homeowner.

1. This permit was issued based on the information provided in the permit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.

The approximate cost of the services based on damages known at the time this contract is entered is $ 6,359.38 determined by State Farm
insurance. Monarch will perform the scrape of work approved by the insurance company. Monarch will supplement for items priced low, not included or items needed to pass citylcounty
codes, which may increase initial dollar amount that insurance is paying out Supplemental claims billed by Monarch that are approved by the insurance company for additional work or
increases in costs become a part of this contract. The final scope and price agreed on between the insurance company and Monarch shall become the final contract price for the insurance
approved scope. Customer will be responsible to pay for any upgrades; deductible, non-recoverable deprecation or additional work authorized by the customer that the
insurance company determines is not covered within the scope of coverage provided by your insurance policy.
Approximate Dates of Service: Sep 15th these dates of service are subject to change due to unusual delays in transportation of materials,
payment delays, insurance approval, ordinary weather delays, or by any othercauses beyond reasonable control of Monarch. Monarch will provide communication to customer
to show tentative dates.
Notice: If the property owner plans to use proceeds of a property and casualty insurance policy issued pursuant to part 1 of article 4:10, C.R.S., to pay for the roofing work, pursuant to
section 6-22-105, the roofing contractor cannot pay, waive, rebate, or promise to pay, waive, or rebate all or part of any insurance deductible applicable to the insurance claim for payment
on the covered residential property.

Notice
THIS CONTRACT IS BETWEEN MONARCH CONSTRUCTION
(-THE COMPANY") AND THE CUSTOMER(S) (THE "CUSTOMER'
WILL BE SUBJECT TO ALL APPROPRIATE LAWS, REGULATIONS
AND ORDINANCES, AND THE FLOOWING SPECIAL TERMS AND
CONDITIONS:
1. This contract and pricing is subject to the approval of the
company's management without exception.
2. All checks for payment must be made payable to
MONARCH CONSTRUCTION & ROOFING, there will be no
exceptions. Customer will still be liable to Monarch
Construction for payment of this contract if check is made
out to any other entity or person.
3. If any provision of this agreement should be held to be
invalid or unenforceable, the validity and enforceability of the
remaining provisions of this agreement shall not be affected.
4. The customer agrees that the equity in this property is
security for the contract.
5. The company shall not be liable for the failure of
performance due to labor controversies, strikes, fire,
weather, and inability to obtain material from usual sources,
acts of god, war, or any other circumstances beyond the
control of the company.
6. If the customer defaults in payment of this contract, charges
shall be added from the date of the default rate of one and
one half percent per month (18% per annum) with a
minimum charge of $2.00 per month. If placed in the hands
of an attorney for collection, company shall be entitled to
collect all attorney's fees, legal filing fees, and cost
expended in enforcing the collection of any charges payable
under the contract.
7. You may cancel the contract and obtain a full refund of any
deposit within seventy-two hours after entering the contract.
In addition, you may rescind a roofing contract pursuant to
C.R.S. 6-22-104.
8. If this contract is cancelled by the customer later than
seventy-two hours from receipt of the written notice
identified in 6-22-104, the customer shall pay the company
twenty five percent of the contract price as consulting
services rendered or liquidated fees. The company agrees
this amount is a reasonable and just compensation for
cancellation and the efforts that the company expended in
the insurance approval process.
9. If material is reordered or restocked because of cancellation
by the customer other than permitted by 6-22-104, the
company will charge a restocking fee equal to twenty five
percent (25%) of the contract price.
10. The contract does not include the replacement of
deteriorated decking, roof jacks, ventilators, flashing or other
materials unless included in the contract, work required will
then been charged on a time and material basis.
11. The company's limited workmanship warranty 5 yrs. covers
the labor to correct problems with the installation of the roof.
The warranty does not cover damage to roofs caused by
lighting, gale 50mph winds, hurricane, tornado, hail storm,
impact of foreign objects, war, or damage to roofs due to
settlement, distortion, failure or cracking of the roof deck,
walls or the foundation of a building. All other construction
warranties besides roofing are 1 yr. The company will
provide you with a written warranty upon receipt and
verification of your payment in full for this agreement.

12. The company is not responsible for any damage below the
roof due to leaks caused by excessive wind over 50mph, ice
damage or hail during the period of the warranty.
13. The company is not liable for any structural movement of the
foundation or walls, settling, cracks in drywall, cracking, and
breaking of the driveway during the roofing or loading
processes, oil droplets in driveway, minor scratching or
denting of rain gutters, damage to flowers, minor broken
branches on plants or shrubbery.
14. All pictures, wall hangings, lights, and collectibles both
indoors and outdoors MUST be secured by homeowner. Any
damage to personal items during installation will not be the
responsibility of the company. The company will also not be
responsible for nail damage to people, animals, tires, pets,
or property during installation. Pets should not be left
unattended during the installation.
15. Any alteration or deviation from the scope of work involving
extra cost will be executed only upon customer approval,
and will become an extra charge over and above the
estimate.
16. This contract and labor warranty shall not be transferable
unless specified in writing.
17. The customer will not receive a labor, material, or
manufacturer warranty if the contract is not paid in full.
18. Any discounts, sign allowance, referral fees, etc. will be
revoked if account is past due or contract is cancelled.
19. This contract constitutes the entire understanding of the
parties, and no other understanding, collateral or otherwise,
shall be binding unless in writing signed by both panties.
20. This agreement is composed of front and back page
materials incorporated herein by reference and shall be
considered the entire agreement by the parties. Any
representation, statements, or other communications not
written in this contract are agreed to be immaterial, and not
relied on by either party, and do not survive he execution of
this contract.
21. All excess materials belong to the company.
22. The customer has sole responsibility to provide safe-
keeping areas for pets. Company shall not be responsible
for gates left open during or after company's work.
23. The final full payment shall not be delayed while waitino
for the city, county or other building inspection
department to inspect the work.
24. Customer acknowledges company is as a general contractor
and as such will be entitled to 10% profit and 10% overhead
if approved by insurance company as part of the claim.
25. Customer is solely responsible for homeowners association
(covenant approval) of roofing products and colors.
26. Company is not responsible for satellite dish realignment.
27. The company reserves the right to cancel this contract at
any time at which time all outstanding invoices are due for
all completed portions of work.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.
