Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped

House Shingles:
Limited Lifetime Shingles Certain Teed Landmark:
Remove the existing layer of shingles down to the existing decking, and dispose of all
debris into dumpster set in the driveway. Inspect existing decking for any deterioration or
spaced sheeting, and replace as needed for an additional $45.00 per sheet. Install ice and water
shield (on all eaves and valleys to prevent ice damming per city code requirements. Install
synthetic felt over the balance of the roof area with plastic caps. Install colored 2X4 drip metal
and 2X2 rake edge on the entire perimeter. Install all needed side wall step flashing, head wall,
back flash, and counter flashing on the sidewalls. Install shingle starter on all eaves and rakes.
Install Certain Teed Landmark Architectural Dimensional Shingles (Resawn Shake) (38 SQ)
— using a six -nail pattern to meet high wind requirements. Install new pipe jacks on all
plumbing pipes. Install new pitched roof jack, rain cap, and storm collar. Install roof vents for
proper ventilation to meet city/manufacture requirements. Install proper ridge cap shingles. Seal
and paint all roof top penetrations to complete.

Service Total: $11,474.00 + permit fees
Payments to be made as follows: Final payment needs to be paid in full within 10 days
from day of completion or late charges will apply. If payment is not received within this
time 18% Monthly Rate will be added. A fee of $75.00 will be assessed for all returned
checks and all denied credit card payments. A 3% convenience charge applies to all credit
card payments. This proposal is subject to the approved credit of the purchaser. It is
valid until August 28`h, 2017 due to continuous price increases from our suppliers, and is
subject to change or withdrawal after that date.
Terms and Conditions: The above proposal, if accepted as a contract or accepted on any other contract form,
incorporates all provisions stated herein. If any legal action arises from this statement, the unsuccessful party shall
pay to the prevailing party all costs and expenses, including attorney's fees and court cost, so incurred by the
prevailing party and such items shall be made a party of any judgment rendered for nonpayment.
Reasonable Control: The Company shall not be responsible for damage or delay (weather or wind) due to
considerations beyond its reasonable control including the availability of all materials. Substitutions may be
required. Additions or deviations shall be considered as a change to the contract. All materials shall be applied
free of defects. The Company is not responsible for reasonable wear and tear to driveways, walks, lawns, shrubs or
other vegetation caused by movement of trucks, men, equipment, materials and debris. All work areas will be
water tight at the end of each work day. We will have emergency contact info for 24 hour service for the duration
of the job. There will be an on-site foreman available for any questions or concerns during work hours. All debris
will be cleaned up on daily basis thoroughly and a magnet sweep of all driveways, sidewalks, and adjacent
landscaping. PRG will be responsible for disposal of all existing materials being removed. All work will be
completed by experienced craftsman and supervised by a foreman to assure a quality installation. All job sites are
supplied with proper safety equipment (harness, cones, flags, first aid kit, fire extinguisher, etc ...).
Facilities: The Accepting Party agrees to provide the Company with sufficient storage room for all materials,
trailers, and staging area for the duration of the job.
Working Hours: will be between 7:30 am — 6:30 pm Mon — Fri, & 9:00 am — 5:00 pm Sat
Solar Panel: Any roof sections with solar panels installed are not warrantable.

Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND

REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
