1. This permit was issued based on the information provided in thepermit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, po plans

2. This. permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building

requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of

5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required

1.0 EXECUTIVE SUMMARY
Foothills Environmental, Inc. (FEI) conducted a limited asbestos survey of materials that may be disturbed
during planned methamphetamine decontamination activities for the residential duplex unit located at 3020
Gray Street in Wheat Ridge, Colorado. A preliminary assessment performed for methamphetamine at the
property concluded that the entire interior of the residence and crawlspace of the residence are considered
contaminated above the state cleanup standards. This limited inspection was limited to the main level living
space portion of the home. The crawlspace portion of the home was inspected during a previous separate
limited inspection performed by FEI. Planned decontamination of the methamphetamine affected property
may include disturbance of textured plaster walls, drywall, sheet vinyl flooring, and cove base with mastic
within the living space portions residence. Ronald Crandall, an Asbestos Inspector certified by the
Colorado Department of Public Health & Environment (CDPHE), conducted the this limited ACM survey.
The Environmental Protection Agency (EPA) defines ACM as a material containing greater than 1%
asbestos. Friable plaster walls were sampled as part of this limited asbestos survey. A friable material is a
material that when dry may be crumbled, pulverized, or reduced to powder by hand pressure. Because
friable materials are more easily damaged and more likely to release fibers into the air, they are handled
differently than non -friable ACM.
Random bulk samples were collected by an EPA and Colorado Department of Public Health (CDPHE)
certified asbestos inspector to determine the asbestos content of the textured plaster walls, drywall, sheet
vinyl flooring, and cove base materials that may be disturbed during methamphetamine decontamination.
This limited asbestos survey was conducted in general accordance to the guidelines published as the
Environmental Protection Agency's Final Rule: Title II of the Toxic Substances Control Act (TSCA), 15
USC, Sections 2641 through 2654 or in compliance with 40 CFR, Part 763 and the Colorado Department
of Public Health and Environment (CDPHE) Regulation #8.
The following materials contain greater than I% asbestos;
• Approximately 154 square feet of "damaged" vinyl sheet floor tile and associated mastics present
in the Kitchen contains 5% chrysotile asbestos in black mastic and 2% chrysotile asbestos in the
red vinyl sheet flooring.
• Approximately 25 square feet of black mastic associated with the brown vinyl cove base located
on the south side of the Kitchen contains 5% chrysotile asbestos in black mastic.
1.1 Introduction
At your request, on June 25, 2018, Foothills Environmental Inc. (FEI) conducted a limited asbestos
inspection of suspect building materials that may be impacted during planned methamphetamine
decontamination activities of specified areas of the north duplex unit located at 3020 Gray St., in Wheat
Ridge, CO. At your request, asbestos sampling was limited to textured walls, sheet vinyl flooring and cove
base that may be impacted during methamphetamine decontamination activities. Mr. Ronald Crandall (Cert
#20438), asbestos inspector certified by the EPA and the Colorado Department of Public Health &
Environment (CDPHE), conducted the asbestos inspection. Aerobiology Associates Inc., an independent
laboratory accredited by the National Voluntary Accreditation Program (NVLAP) and the American
Industrial Hygiene Association (AIHA), analyzed the samples utilizing Polarized Light Microscopy (PLM).
1.2 Scope of Work
The combined goals of sampling and visual assessment were to:

Identify suspect asbestos -containing material (ACM) and take samples as needed in the area of
metbamphetamine decontamination and document the location, condition, friability and quantity
of each identified material.
2. Compile sample data information, observations obtained from the site visits, conclusions and
recommendations into a report form.
Make appropriate recommendations on how to approach each material identified as an ACM prior
to methamphetamine decontamination.
1.3 Standard Bulk Sampling and Analytical Procedures
The inspection was completed by separating materials into Homogeneous Areas. A homogeneous area
(material) is defined as an area containing a material that appears similar throughout with regard to color,
texture, and date of application. Individual systems that were inspected, but not suspected to contain
asbestos, are not included in this report. Such systems include metal, concrete, carpet, fiberglass, plastic,
and wood products. From the list of suspect homogeneous areas, a physical assessment was performed for
each material on the list. A physical assessment includes evaluating the condition, friability, and amount
of damage of each material. By definition, "friable" materials are those that can be crumbled or reduced to
powder by hand pressure when dry. Each material on the list was further classified into one of three
categories, which have specific sampling requirements for each category.
Surfacing Materials: Refers to spray or troweled applied surfaces such as plaster ceilings and
walls, fireproofing, textured paints, textured plasters, and spray -applied
acoustical surfaces.
Thermal System Insulation: Refers to insulation used to inhibit heat gain or loss on pipes, boilers,
tanks, ducts, and various other building components.
Miscellaneous Materials: Refers to friable and non -friable products and materials that do not fit in
any of the above two (2) categories such as resilient floor covering,
baseboards, mastics, adhesives, roofing material, caulking, glazing, and
siding. This category also contains wallboard, joint compound, and ceiling
tiles.
The condition of suspect materials was evaluated as "good", "damaged", or "significantly damaged" using
the following parameters:
Good- material with no visible damage or deterioration or showing only very limited damage or
deterioration.
Damaged- material which has deteriorated or sustained physical injury such that the internal
structure (cohesion) of the material is inadequate or, if applicable, which has delaminated such that
the bond to the substrate (adhesion) is inadequate or which for any other reason lacks fiber cohesion
or adhesion qualities. Damaged material are those that are <10% scattered or <25% localized.

Significantly Damaged- material which has extensive and severe damage. Significantly damaged
materials are those that are >10% scattered or > 25% localized.
Bulk samples collected were analyzed utilizing the Environmental Protection Agency's Method for the
Detection of Asbestos in Bulk Samples (EPA 600/R-93/116, June 1993) and the McCrone Research
Institute's The Asbestos Particle Atlas as method reference.
1.4 Building Description
3020 Gray Street is located on the north half of the duplex residential unit and is a wood framed brick
masonry structure on concrete foundation with a crawlspace. FEI previously performed a limited inspection
of the crawlspace, HVAC system air handler unit and ductwork (FEI, May, 21, 2018. Crawlspace Limited

4.0 ASBESTOS RECOMMENDED ACTIONS
Asbestos -Containing Materials — General
Removal, in accordance with the Colorado Department of Public Health and Environment's
(CDPHE) Regulation No. 8, is required of materials with an asbestos content greater than one
percent (1%) that are friable or will be made friable during renovation or demolition activities.
Friable means that the material, when dry may be crumbled, pulverized, or reduced to powder by
hand pressure.
The National Emission Standard for Hazardous Air Pollutants (NESHAP) regulations set forth by
the U.S. Environmental Protection Agency control asbestos emissions from renovation and
demolition activities.
The Occupational Safety and Health Administration (OSHA) Construction Asbestos Standard 29
CFR 1926.1101 contains work practice and engineering control requirements and prohibitions that
must be observed regardless of the percentage of asbestos in the installed construction materials.
The standard also has exposure -based requirements consisting of a 0.1 fiber/ cc 8 hour Time
Weighted Average (TWA) Personal Exposure Limit (PEL) and a 1 fiber/ cc 30 -minute excursion
limit.
The following sections describe materials that were identified during this inspection and
recommended actions prior to renovation or demolition.
4.1 Friable ACM
The following friable ACM was identified during the inspection:
• Vinyl sheet flooring and associated black mastic
Friable ACM must be removed prior to renovation or demolition. Although the red vinyl sheet
flooring is non -friable while intact, this material is significantly damaged and
methamphetamine decontamination may require removal of the vinyl flooring which will
render it friable during removal.
It should be noted that the total square footage of asbestos containing vinyl flooring and
associated black mastic exceeds the trigger levels (>32 square feet), and removal of the
vinyl flooring will require a permitted full containment abatement project with visual
inspection and PCM air clearance.
4.2 Non -friable ACM
The following non -friable ACM were identified during the inspection:
• black mastic associated with brown vinyl cove base

4.3 Materials Containing 1% or Less Asbestos (Trace)
• Approximately 2,670 square feet of "damaged" plaster walls and ceilings throughout
residence contains less than 0.25 percent chrysotile asbestos.
For Trace Amounts of Asbestos

The employer still must follow the requirements in paragraphs (g)(1) [except (g)(1)(i)], (g)(2) and
(g)(3) that describe engineering and work practice controls operation as well as prohibitions that
must be observed regardless of the percentage of asbestos in the installed construction materials.
The standard also has exposure -based requirements consisting of a 0.1 fiber/ cc 8 hour Time
Weighted Average (TWA) Personal Exposure Limit (PEL) and a I fiber/ cc 30 -minute excursion
limit.
5.0 LIMITATIONS
This report describes the locations and conditions of suspect materials identified in the facilities
during the inspections. FEI represents that our services are performed within the limits prescribed
by applicable regulations and in a manner consistent with the level of care and skill ordinarily
exercised by other professional consultants under similar circumstances. No other representation
is made to the client, expressed or implied, and no warranty or guarantee is included or intended.

The subject property is located at 3020 Gray Street in Wheat Ridge, Colorado (residence). Foothills
Environmental, Inc. (FEI) was contracted by Morey Grebb of Grebb West 2, LLC. This Preliminary
Assessment (PA) covers only the 3020 Gray Street half of the duplex. Mr. Grebb requested the residence
to be tested for methamphetamine based on the condition of the property once the tenant vacated the
property.
FEI conducted sampling for a Preliminary Assessment of the duplex apartment on April 27, 2018. The
analytical results verified the presence of methamphetamine at concentrations greater than 0.5 ug/100
cm' throughout the residence. The Colorado Department of Public Health and Environment (CDPHE)
classifies properties with methamphetamine surface contamination exceeding 0.5 ug/100 cm' as meth -
affected properties (6 CCR 1014-3, Regulations Pertaining to the Cleanup of Methamphetamine -Affected
Properties). The analytical results verified the presence of methamphetamine at concentrations greater
than 4 ug/100 cmZ in the crawlspace (limited exposure area) of the residence. The CDPHE classifies limited
exposure areas (i.e., attics and crawl spaces) of properties with methamphetamine surface contamination
exceeding 4 ug/100 cm' as meth -affected. The analytical results revealed methamphetamine
concentrations less than 4 ug/100 cmZ in the attic space portion of the residence.
The duplex structure on the property was built in approximately 1950. The 3020 Gray Street residence is
comprised of two (2) bedroom, a Bathroom, a Kitchen, a Living Room, an attached Garage, a Crawlspace
and an Attic. The residence is equipped with a forced air heating and air conditioning system. The
Bathroom exhaust fan is ducted through the roof of the residence. The furnace and hot water heater
exhaust ducts merge and exhaust through the roof of the residence. The total square footage of the
residence is 624 ftZ. The square footage of the attached garage is 252 ft2. The property is connected to
city water and sewer.

Based on the analytical results, the entire interior of the single family residence, the HVAC system, the
exhaust ducts/vents, crawlspace, and attached garage are considered contaminated where
methamphetamine levels are above the CDPHE cleanup standard. The attic space was the only portion
of the residence that was not considered contaminated above the CDPHE cleanup standard. FEI
recommends that all interior rooms, the furnace and HVAC ducts, the crawlspace, the attached garage,
all appliances and all contents are either cleaned and re -tested or disposed of by a decontamination
contractor. No personnel are allowed to enter the residence including realtors, maintenance personnel,
contractors or others without the proper training. The decontamination and cleaning must be performed
by a licensed methamphetamine contractor in Colorado with certified workers. A clearance assessment
for the areas requiring decontamination including all rooms within the residence, the crawlspace, HVAC
system, the attached garage, and all remaining appliances will be required once a contractor has
decontaminated the residence.
7 RECOMMENDED SCOPE OF WORK
Based on the samples collected throughout the property, the following recommended scope of work
should be completed.
All contents throughout the interior rooms, crawlspace and attached garage are considered
contaminated and should either be disposed of or decontaminated and sampled.
All suspect asbestos containing building materials that may be disturbed as part of the decontamination
process should be tested for asbestos before work begins. All suspect asbestos containing building
materials must be assumed to contain asbestos until sampling proves otherwise.
Decontaminate all rooms, the attached garage and the crawlspace throughout the residence.
Two (2) inches of soil shall be removed from the soil floor of the crawlspace. Suspect asbestos
containing duct wrap was observed hanging loose from the HVAC system ductwork and loose pieces of
the duct wrap were observed on the soil floor.
Decontaminate or remove and dispose of the furnace and all ductwork. Clean all associated wall cavities
used as cold air returns for the HVAC system.
Remove and dispose of the Bathroom fan and exhaust vent ductwork.
Remove and dispose of the water heater exhaust vent ductwork.
8 SAMPLE METHODOLOGY
Methamphetamine Wipe Samples
All methamphetamine wipe samples were collected using 2x2 inch polyester blend gauze pads. A new
100 cm2 template was affixed to all surfaces for sampling. Powder free nitrile gloves were donned prior
to collecting each sample. Gloves were doffed after collecting each sample. The same pair of gloves were
worn when collecting composite samples only. The gauze pads were dampened using isopropanol to
enhance collection efficiency. The property was vacant on April 27th, 2018 and was not painted prior to
testing, so isopropanol was used as a wetting agent for all painted surfaces. Samples were collected in
accordance with 6 CCR 1014-3 Part 1, Section 6.2.7 through 6.2.11.
Field blanks were collected for this project. The field blanks were prepped and handled exactly as each
sample was prepped and handled. The field blanks were submitted along with all other samples via chain

of custody to Reservoir Environmental, Inc. (REI) located at 5801 Logan Street, Suite 100 in Denver, CO.
The samples were analyzed by Gas Chromatography Mass Spectrometry (GC/MS). REI conducted Quality
Control reporting requirements for matrix blank, matrix duplicate, matrix spike and laboratory control as
required — all were within acceptable range. The Reporting Limit for the analytical method was 0.05 ug.

Consultant Interim Authorization Approval
Dear Andre Gonzalez:
The Hazardous Materials and Waste Management Division of the Colorado Department of Public
Health and Environment (the Department) has reviewed the application for Consultant Interim
Authorization and has determined the application to be complete and in compliance with 6 CCR
1014-3, Regulations Pertaining to the Cleanup of Methamphetamine -Affected Properties (the
Regulations).
The Department approves the application for Consultant Interim Authorization. This interim
authorization approval shall expire on June 15, 2015.
Assessment and sampling of methamphetamine -affected properties may only be conducted by
Consultant Firms that have received interim authorization under Part 2, Section 3.2.4 of the
Regulations. Therefore, this interim authorization may only be used to conduct assessment and
sampling of methamphetamine -affected properties for a Consultant Firm that has received interim
authorization approval from the Department.
