Lutheran Parkway, Suite 100 Wheat Ridge, CO 80033 S & R Project Number: 017070 Dear Mrs. Binkowski, S&R Environmental Consulting, Inc. (S&R) is pleased to provide the results of the bulk samples collected from suspect asbestos -containing materials (ACM) identified at the following address 3455 W. Lutheran Parkway, Suite 100 in Denver, Colorado. The collection of representative bulk samples and delivery of the samples to an independent analytical laboratory was conducted on March 23`d & March 24th, 2017. The work was performed by S&R Asbestos Building Inspector, Torie Chelius, who is certified by the Environmental Protection Agency (EPA) and the Colorado Department of Public Health and Environment (CDPHE). INTRODUCTION S&R understands that Coda Construction was seeking a professional environmental consulting company to collect representative bulk samples of suspect asbestos -containing materials that may be impacted during an upcoming renovation project of the above address. The Suite consists of a front office reception area and back medical rooms. They are combined and renovated in several areas. Samples were submitted to an independent analytical laboratory for asbestos content analysis. S&R understands that a report of findings would be prepared based on the results of the analysis. This survey was conducted in a manner to meet all the requirements to be issued a building permit if needed. SCOPE OF WORK Twenty-one (21) representative bulk samples were collecte lining materials identified throughout the suite. Samples were subllysis. Samples were collected throughout the front offices as Th following is a list of suspect building materials identified thn ..�.. rem tested: ��eatFt,d9e • Drywall and Surfacing Materials • Flooring Materials and Associated Mastics • Ceiling Tiles • Miscellaneous Materials ANALYTICAL PROCEDURES The bulk samples collected from suspect asbestos -containing materials were delivered to Reservoirs Environmental, Inc., a National Voluntary Laboratory Accreditation Program (NVLAP) asbestos laboratory located in Denver, Colorado for a rush turnaround time. All bulk samples are archived for two months unless otherwise stipulated by the client. According to the analytical laboratory, the bulk samples were analyzed in accordance with EPA Method 600/R-93/116. Small portions of the sample were placed in Series: E High Dispersion Refractive Index Liquid on a microscope slide. The prepared samples were observed at 100X (power) under polarized light using a McCrone Dispersion Staining Objective. The characteristics of the fibers were compared to the known properties of asbestos fibers for dispersion, color, polarity, distinction and general morphology. Sample content (given by percentage) was made by visual estimates by comparison of asbestos fibers to total materials. RESULTS The US Enviromnental Protection Agency (EPA), the Occupational Safety and Health Administration (OSHA), and the State of Colorado Air Quality Control Commission consider a material as asbestos containing if the reported amount of asbestos is greater than one percent (1%). TACHOMETER RPM readings are taken with a PIONEER PLT5000 contact/noncontact laser tachometer. VOLT/AMP METER A FLUKE 335 TRUE RMS CLAMP METER which measures volts, amps and ohms. Read-out is digital. The samples were submitted to REI Lab, Inc. for analysis via EPA 600/R-93/116 Method using Polarized Light Microscopy. Guidelines used for the inspection were established by the Environmental Protection Agency (EPA) in order to comply with the Air Quality Control Commission Regulation No. 8, Part B "Emission Standards for Asbestos." Field Information was organized as per the AHERA (Asbestos Hazard Emergency Response Act) concept of Homogeneous Area. A Homogeneous Area is defined as a suspect material of similar age, appearance, function, and texture. If damage is extensive enough that the homogeneous areas cannot be defined, samples will be randomly obtained per functional space. Each material was grouped together as a specific Homogeneous Area or obtained from a specific functional space, sampled, and then assessed for condition. The number of samples of surfacing material, such as drywall texture, is dependent on the square footage of the drywall to be removed. Due to the square footage of the textured wall and ceiling material to be removed in the unit, less than 1,000 square feet, the Homogenous areas textured wall materials will require at least three samples of each type according to AHERA. All materials sampled have been slated for demolition. Consequently, invasive techniques may have been utilized to obtain or clear areas of suspect (Asbestos Containing Materials) ACM. Material quantities are approximate as exact amount of demolition may vary depending on a number of factors such as success of dry -out, extent of smoke damage, etc. Consequently, for these types of environments we recommend the contractor verify exact material amounts. Any materials not tested but mentioned in this report are non -suspect materials (wood, metal, plastic, rubber, or glass).
