Subject: Visual structural foundation evaluation for 4590 Brentwood St., Wheat
Ridge, CO.
Dear Mr. Henry,
Per your request, I visited the house at the above referenced address on
April 27, 2021. The purposes of my visits were to observe the foundation
damage reported and provide repair recommendations as required.
Please note: My investigation was based on observation of accessible,
visible items. Issues/concerns concealed by finishes, if any exist, were
not able to be observed, and were therefore not evaluated as part of this
investigation. Issues and repairs, if required, regarding other parts of the
home were not part of the scope of this investigation.
Observations
For purposes of this report, the front of the home faced west. The home
was settled and cracked along the west side, west end of the north side,
and the southwest corner extending east along the south side. In addition,
some additional movement may have occurred at interior locations.
Please note, the repairs recommended herein will not address issues
related to interior bearing wall movement.
4590 Brentwood St., Wheat Ridge, Colorado
Recommendations
1) 1 recommend stabilizing the house with twelve (12) helical piers per
the attached engineering drawings. I do not recommend lifting.
2) 1 recommend monitoring the interior areas for additional movement.
Please note: additional repairs or changes to the proposed repairs
may be required as construction proceeds and reveals the
underlying structure.
3) If applicable, racked doors and windows should be reset, and
cosmetic repairs can be made three (3) months after the structural
repairs are complete.
4) It is very important to maintain proper drainage away from the
house. I do not recommend irrigating within 5 -feet of the
foundation. Maintain a minimum 10 % slope away from the
structure, and direct surface water to an adequately sloped swale.
Grass, plants, trees and shrubs with high watering requirements
should not be located within five feet of the foundation. All gutter
downspouts should have functioning downspout extensions, which
extend at least five feet away from the foundation and discharge in
a manner which allows effluent to flow safely away from the home,
or discharge directly to an underground pipe system with an
appropriate daylight location. Perimeter drains, and sump pumps, if
they exist, should function properly. Proper separation of grade to
top of foundation should be maintained.
I observed the structural condition of the visible portions of the
foundation described above. Other unknown concerns may also exist.
The Lepetsos Group, Inc. makes no representations regarding the
suitability of other areas, including, but not limited to mechanical,
electrical, plumbing, previous foundation repairs, and other systems.
The homeowner should understand additional unforeseen and
unforeseeable issues related to soil heaving and or settlement, and
exposure to unwanted water may cause additional foundation
movement in the future. My evaluation and recommendations are
based on my education, experience and observations made on the
date of my visit. The Lepetsos Group, Inc. makes no representations
regarding the ability of the property to meet current or past building

Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat

REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75% of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-750 of the mid -roof may be complete. Asphalt shingles are required to be
fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.
