Building One reviewed the Building Envelope at the Outlook Clear Creek property, specifically at the 4048
Clear Creek Drive address. The weather resistive barrier was observed at all exterior wall assemblies.
Building One initially noted some inconsistencies, but it appears that all our concerns have been addressed.
It is our understanding that the exterior wall assemblies are in general conformance with the construction
documents and industry standards.

Dear Mrs. Grady,
Per our contract, our office reviewed tendon elongation report dated June 12, 2020 provided by Cole
Garner Geotechnical for the above referenced project. The reports indicate that the special inspector
from Cole Garner observed as the tendons were stressed using the following equipment:
Ram No. PT Jack & Accessories #302
Gauge No. #302 McDaniels, 10,000 PSI, 4"
Pump No. Enerpac, #302, S/N 1403
The equipment was last calibrated on May 15, 2020. The corresponding stressing gauge pressure is
5,460 pounds per square inch when applying 33 kips of tension to the tendon. A copy of the associated
calibration curve was provided to our office for our records and review.
Based on our review of the stressing records, tendons #12A & #12B were not stressed due to
unacceptable consolidation around the stressing end of the tendon. These tendons will need to be
repaired and restressed per the attached repair letter for Garage C, dated June 17, 2020. Our office
recommends that during the placement of the concrete to be consolidated at the anchorage with
vibratory methods.

Dear Mrs. Grady,
Our office recommends that the following steps be taken to remove and repair the post -tensioned slab on grade
when an anchor does not have adequate consolidation at the anchor(s). Our office strongly recommends that
during concrete placement the concrete be consolidated at the anchorages with vibratory methods performed by
the foundation contractor.
1. The slab shall be examined and the locations of the tendon(s) that do not meet elongation requirements
outlined on the construction documents and project specifications be marked.
2. Each tendon that is not within the tolerances stated above should be de -tensioned.
3. The dead and/or live end anchors should be carefully chipped out a sufficient amount to expose the
anchor and remove all unconsolidated concrete. Minimize the amount of concrete removed. All
unconsolidated concrete should be removed and the concrete testing agency shall make a special
inspection to determine that the exposed concrete is sufficiently consolidated and that all unconsolidated
concrete has been removed. Provide a copy of their report to our office for review.
4. Chip down the edges of the concrete in the area removed to create a vertical surface.
5. Clean out all debris and broken aggregate around the perimeter of the chipped out slab.
6. Clean concrete and apply a concrete bonding agent to the vertical surface of the chipped out area prior to
placing new concrete.
7. Fill in chipped out area with concrete having a minimum compressive strength of at least 10,000 psi. The
slump of the concrete should not be greater than four inches and the water cement ratio shall not exceed
0.45.
8. Re -stress the tendons after the concrete has reached a minimum compressive strength of at least 2,500 psi.
9. Provide secondary stressing records to our office for review prior to cutting and packing of the tendon
tails.
For future construction problems such as this can easily be avoided by the use of vibratory methods performed by
the foundation contracture during concrete placement. Additionally, the Construction and Maintenance Manual
for Post -Tensioned Slab -on -Ground Foundations, Third Edition recommends that when tendons are not being
stressed within the tolerances of the contract documents and project specifications that tendon stressing operations
be stopped until the problem can be resolved.

Our office reviewed the stressing record dated June 12, 2020 the tendons #12A & #12B were not stressed
due to unacceptable consolidation around the stressing end of the tendon. The stressing record dated June
30, 2020 was for the stressing of these tendons.
Based on our review of these stressing records, calibration curve, PTI allowances of +/-1/4" elongations for
short tendons, and the knowledge of measurements taken to the nearest 1/8", it is our opinion that the
tendons have been adequately stressed and can be burnt off to a distance of 3/4" inside the exterior surface of
the concrete. Stressing pockets should be packed with grout as outlined in the contract documents.
