Our office strongly recommends that all tendons be located prior to the installation of post installed anchors for current and future construction.: 1. The slab shall be examined and the locations of the broken tendon(s) shall be verified and all tendons shall be verified and marked prior to demolition of the slab to ensure that no additional tendons will be damaged during repair procedures. 2. Carefully chip out the concrete in the designated area noted above a sufficient amount to install a tendon coupler and PVC sleeve as depicted in figure 11.3 of the Construction and Maintenance Manual for Post -Tensioned Slab -on - Ground Foundations, Third Edition. Do not remove any concrete outside of the noted area without specific permission from the engineer of record and minimize the area of concrete removed. We anticipate that an approximate 2'-0" x 2'-0" area of concrete will require removal. If more than the aforementioned area of concrete is required to be removed contact our office for additional recommendations. 3. Chip down the edges of the concrete in the area removed to create a vertical surface. 4. Clean out all debris and broken aggregate around the perimeter of the chipped out slab. 5. A new 7/16" diameter tendon should be installed though the existing pocket former and be extended to the location where the original tendon broke. The new tendon and existing tendon should be spliced together with a coupler following all manufacturer directions. 6. Place number 4 (grade 60) epoxy dowels at sixteen inches on center. Embed bars four inches into the existing slab, minimum. The epoxy dowels should be centered in the slab depth and extend to the opposite side of the chipped out area. At the opposite side of the chipped out area, the epoxy dowel should have a four -inch hook. Provide dowels in both directions. 7. Sleeve all plumbing penetrations between the top of concrete and sub grade with a minimum of %Z" foam material. 8. Clean concrete and apply a concrete bonding agent to the vertical surface of the chipped out area prior to placing new concrete. 9. Contact the special inspector for a tendon repair inspection with all steel in place and all repairs made prior to placing new concrete. 10. Fill in chipped out area with concrete having a minimum compressive strength of 4000 psi. The slump of the concrete should not be greater than four inches and the water cement ratio shall not exceed 0.45. 11. Obtain a gauge calibration chart from the tendon supplier indicating the proper gauge pressure for a 7/16" diameter low relaxation strand. Provide this chart along with the special inspector's tendon elongation inspection report to the engineer of record prior to cutting the tendon tail. 12.
