NOTICE TO SURVEYOR:
Attach the approved Construction Control Plan to this form and follow
the procedure below:
1. Within the area of interest or project "Phase", include the As -
Built ("AB") setback distances and elevations at the required
locations as defined on the Construction Control Plan (`CCP").
2. Bubble or clearly highlight the area(s) where the current
measurements were collected and added to the CCP.
3. This Certification form and associated Plan Sheets must be
returned to the City of Wheat Ridge Building Division at 7500
W. 29' Avenue, Wheat Ridge, CO 80033 (303.235.2876) for
review and approval of the Phase prior to commencement of
further construction work on the project.
4. Following approval the next construction Phase can then
commence.
5. Steps 1-3 shall be performed for each Phase and the additional
AB information included on the previous CCP, until all Phases
have been completed.
6. Upon completion of all construction, the resultant CCP will
contain ALL As -Built elevation information for ALL Phases of
the project.
7. The CCP shall then be submitted for review and approval prior to
issuance of the first Certificate of Occupancy for the final
building in the development.

Planning Conditions:
-All lighting shall be in conformance with Section 26-503 of the Zoning and Development
Code with fixtures fully shielded so that the source of the illumination is not visible
from adjacent properties and streets. All. lighting shall be downcast.
-All parking and driving areas shall be in conformance with Section 26-501 of the Zoning
and Development Code.
-Site signage will require separate permits.
-All landscaping shall be in conformance with Section 26-502 of the Zoning and Development
Code. All landscaping shall be installed in accordance with the approved landscape plan
prior to C.O., a landscape escrow agreement with financial guarantee in the amount of 125%
of the cost of the installation shall be required.
-Owner/Contractor is responsible for locating property lines and constructing improvements
according to the approved plan and required development standards. The City is not
responsible for inaccurate information submitted within the plan set and any construction
errors resulting from inaccurate information.
-Paint colors and brick shall match primary building colors
-The community building shall be a maximum of 15' in height as measured from average
finished grade to midpoint of roof pitch
-Approval of this permit shall not be construed as approval of site work, except as it
pertains to the community building. A separate site work permit application will. be

On behalf of the Clear Creek Valley Water and Sanitation District (CCVWSD), Martin/Martin Inc., acting as the District's
Consulting Engineer, offers the following in response to the site plan received March 18, 2020, concerning a proposed
community building (2100 sf) at the above referenced address. The Clear Creek Valley Water and Sanitation District has
no objections with the proposed development conditional on the items identified herein are fully addressed. The
following are general requirements for sanitary sewer service. Rules, Regulations and Standards of the District must be
complied with at all times.
The subject lot appears to be entirely within the service and boundary area of the Clear Creek Valley Water and
Sanitation District. Treatment of sewage generated within the Clear Creek Valley Water and Sanitation District is
provided by Metro Wastewater Reclamation District.
Existing District Sanitary Sewer Mainline(s)
Clear Creek Valley Water and Sanitation District has the following sanitary sewer mains in the vicinity of the proposed
property:
• 8 -inch mainline running south to north, east of the referenced property within Garrison Street.
It appears the property may be serviced by gravity flow; however, the owner/developer is responsible for determining
horizontal locations and vertical depths of the existing sanitary sewer main(s) and potential conflicting utilities, to verify
if service and gravity flow is feasible or if a private individual sewage ejector (lift station) is required. The issuance of a
sewer tap permit by the District does not in any way guarantee that the property can be served by gravity flow. No
underdrain or storm sump pump connections to District facilities are allowed.
Utility Locates
All projects must meet Colorado Revised Statue Title 9 for proper notification to the Notification Association by the
Developer's professional engineer. It is also the owner's responsibility to include electronic locators on all new
underground facilities, including service lines up to the structure or building being served. Submit details for tracer
wire and associated test stations.
Proposed Development
The proposed building requires a water meter initially sized at 1" per plan, the District will need to review and approve
plans prior to construction. If a water meter size increase takes place, the District must be informed immediately, and
appropriate sanitary tap fees must be paid prior to acceptance. In addition, the District will require review of the
alignment and service connection to the mainline. Please provide a letter from the water service provider with a
determination of the final water meter size.

Sanitary Sewer Service
The service line appears to be coming out of the east side of the building. A site plan alignment and elevations are
needed for review, showing connection to the District main. Invert and rim elevations are needed on each side of the
proposed service connection to verify connection will be at 10:00 or 2:00 position. If the service is 6" or larger in size, a
manhole connection is required. For a 4" connection a saddle wye tap is required with core into the main.
Sand/Oil Interceptor
For multi -family, commercial, and retail development where sand/oil interceptors are required, proper sizing per
Uniform Plumbing Code (UPC) is to be submitted for review and approval. A Sand Oil Interceptor is required to
accommodate the laundry facility and the maintenance workshop. Please submit sand/oil Interceptor sizing
calculations as well as a product cut sheet for district review. Martin/Martin, on behalf of the Clear Creek Valley Water
and Sanitation District, will review the drawings and calculations for compliance with the District's Rules and
Regulations. Additional federal, state and local jurisdictional requirements may apply.
The proposed construction documents require re -submittal for review and acceptance. The following comments are
initial high view comments associated with the interceptor and are not all inclusive:
• Design and sizing calculations must be provided to show sufficient volume of a sand oil interceptor is being
provided according to the District sizing and UPC standards. Please provide a certified copy from the design
engineer of said sizing calculations for District Records.
• A District representative shall observe the installation of each unit prior to backfilling and placing in service.
• Each facility shall maintain records of their operation and maintenance of the unit(s).
• The District will continue to periodically observe these units to ensure that they are operated properly and
receive the appropriate maintenance at the owner's cost/expense. Any fines associated with non-compliance
of the standards and maintenance of the unit will also be the responsibility of the owner.
• Submit layout and elevation alignment of domestic connection after interceptor.
Flow Study (CCVWSD)
A flow study may be required to verify if the development will cause any downstream line segments to exceed
allowable capacities. Developer is responsible for the flow study (if required) and any improvements to eliminate found
capacity issues caused by the development.
Costs
All costs involved are to be deposited in advance—engineering, reviews, design, legal, construction, observation and
inspections — are the responsibility of the Owner/Developer at the then current rate fee structure. Please be aware of
the following: 1. Deposit must be made prior to reviews and further coordination. 2. Application for tap fees and
development fees is required per standards. 3. All fees are due prior to connection.
A review fee in the amount of $1,500 is required to be deposited to Clear Creek Valley Water and Sanitation District
to continue the review process. Please inform us when you have sent the deposit and the District will confirm
receipt for us.
A water provider letter verifying water meter sizing for all units/buildings will be required prior to a tap application
completion. Full addresses of each sanitary service are also required. A tap application must be submitted to the
District for review. If credits for SFRE's are being requested for existing units(s)/buildings, the Developer must provide
existing addresses, meter size, and if applicable, abandon the sanitary tap at the mainline (District observation and fee
is required) prior to final new connection acceptance. A minimum 72 -hour notice prior to construction is required
following District receipt of all fees. These fee amounts can be paid directly to the District Office prior to connection,
payable to Clear Creek Valley Water and Sanitation District, which also collects Metro Wastewater's "connection fees".
